临床标本分离的细菌分布及耐药性分析

【摘要】 目的 了解本院近年来临床送检标本的细菌分布及耐药情况。 方法 按《全国临床检验操作规程》的要求接种 ,分纯后用BDSeptor或VitekAms仪进行鉴定和药敏试验 ,药敏结果用Whonet 4软件进行统计分析。结果 共分离出菌株 2 2 89株 ,其中凝固酶阴性葡萄球菌 (CNS)占所有分离菌株的 17 7% ,铜绿假单胞菌占 15 6 % ,大肠埃希菌占 13 5 % ,肠球菌占 10 8% ,金黄色葡萄球菌占 10 3% ,克雷伯菌占 6 3% ,不动杆菌占 5 2 % ,其它菌种占 2 0 5 %。革兰阴性杆菌对亚胺培南的耐药率最低 ,球菌中未检出万古霉素的耐药菌株 ,ICU病房患者标本中分离菌株多重耐药严重。 结论 临床在治疗患者时不能只凭经验用药 ,而应该积极送检相应的标本进行培养 ,以细菌检验报告为依据进行治疗 ,否则会导致更严重的耐药。

【Abstract】 Objective To investigate the distribution and the drug resistance instance of the pathogenic floras isolated from clinical specimen of the First Affilliated Hospital of Sun Yat-sen University of Medical Sciences. Methods Specimen were inoculated with the methods descriped by the National clinical laboratory operation regulations,the purified floras were identified and the microbial sensitivity tests were made with BD Septor or Vitek Ams instrument,the results of microbial sensitivity tests were analysed and stated with Whonet software. Results Totally 2 289stains were isolated,the proportion of Staphylococcus Coagulase negative,Pseudomonas aerutinosa,Escherichia coli,Enterococcus,Staphylococcus aurus,Acinectobacter and others was 17.7%,15.6%,13.5%,10.8%,10.3%,6.3%,5.2%,20.5% percent respectively.Most Gram-Negative bacilli were sensitive to Imipenem,no Vancomycin resistant Gram-Positive coccus was isolated,most pathogenic bacteria isolated from the specimen of Intensive Care Unit represented mutiple resistance. Conclusion The increasing resistant tendency suggests that we could not make medication based on experience but cultivate corresponding specimen to get the true resistant message for medication,or more and more severely resistant bacteria will be produced.
