【摘要】 目的　探讨动脉旁路术后并发移植物感染、狭窄和闭塞以及吻合口假性动脉瘤的治疗。方法　对 1 986年以来作者经治的 86例旁路术后 1 1例并发移植物感染、狭窄闭塞和假性动脉瘤进行回顾性分析。结果　移植物感染致病菌属假单胞菌属、肠杆菌属和葡萄菌属各 1例 ,手术治愈 2例 ,术后截肢 1例。阻塞者为急性血栓形成 1例 ,狭窄 2例 ,增生性闭塞 4例 ,其中 3例远端病变发展 ,药物维持治疗病情好转 2例 ,恶化 1例。手术治愈 3例 ,手术失败截肢后死亡 1例。吻合口假性动脉瘤 1例 ,行瘤壁切除、裂口缝合治愈。结论　三种并发症均面临截肢和死亡的威胁。移植物感染和假性动脉瘤一经明确诊断 ,均应立即手术。更多还原

【Abstract】 Objective To explore the treatment of graft infection, stenosis or occlusion after prosthetic bypass. Methods A retrospective analysis was made for 86 patients with neopathy since 1986, including graft infection, stenosis or occlusion and pulstatile hematoma. Results The pathogen of graft infection is Pseudomonas, Bacillus coli or Staphyloccus separately. In the three cases of graft infection, cure of operation was for two, amputation for one. In the seven cases of obstrution, one was acute thrombosis, stenosis for two, proliferative obstruction for four, including distal lesion for three. The treatment of pharmacotherapy for three, two was improved, another was deteriorative. Cure of operation for three, but one died from amputation. Pulstatile hematoma was for one, which was cured by resection. Conclusions Three kinds of neopathy threatened patients to face ampulation and death. If the diagnosis of graft infection, stenosis and pulstatile hematoma is definite, the operation must be adopted immediately.更多还原
