【摘要】 目的　研究CagA阳性及CagA阴性幽门螺杆菌 (Hp)感染的慢性胃炎不同年龄组与肠化生之间的关系。方法　从 1999年 10月～ 2 0 0 0年 5月收集了Hp感染的慢性胃炎 491例 ,分组情况为 <30岁、30～ 40岁、40～ 5 0岁、5 0～ 6 0岁、6 0～ 70岁及 >70岁。用多聚酶链反应 (PCR)技术扩增慢性胃炎黏膜中分离的Hp菌株的CagA基因 ;将所取慢性胃炎患者黏膜用HID -ABpH2 .5法及ABpH2 .5 -PAS法染色判断不同类型肠化生。结果　用趋势检验分析表明 ,在Hp感染的慢性胃炎患者CagA阳性发生率不随年龄增长而增加 (P >0 .0 5 ) ,但40～ 5 0岁为一峰值 ;CagA阳性及CagA阴性HP感染的慢性胃炎患者肠化生的发生率均随年龄增长而增加 (P<0 .0 5 )。用卡方检验分析表明 ,CagA阳性Hp感染的慢性胃炎患者更易发生肠化生 (P <0 .0 5 ) ,但与不同类型肠化生的发生率无关 (P >0 .0 5 )。结论　CagA基因与肠化生的发生有关 ,但与分类无关 ;同时还发现肠化生的发生为多因素影响的结果。更多还原

【Abstract】 Objective To study the relationship between intestinal metaplasia and Hp infected gastritis with and without cagA in different age groups. Methods 461 patients were collected from October 1999 to May 2000. The age range was 26~82 years and the patients were allocated into six age groups. PCR was carried out to detect cagA.HID-AB pH 2.5 and AB pH-PAS methods were used to determine the types of intestinal metaplasia. Results Among the patients with Hp-infected gastritis,cagA did not increase with increase of age, and in both cagA positive and negative patients the incidence of metaplasia increased with increase of age. CagA positive patients were more prone to have intestinal metaplasia,but there was no relationship between cagA and types of intestinal metaplasia. Conclusion CagA is related to the occurrence of intestinal metaplasia, but not related to the types of intestinal metaplasia. The occurrence of intestinal metaplasioa is probably multifactorial.更多还原
