原发性高血压患者体脂分布与胰岛素抵抗的关系

【摘要】 采用放射免疫分析法测定 2 1例原发性高血压 (EH)伴肥胖者 ,19例单纯性肥胖及 16例正常体重者的空腹及口服 75 g葡萄糖耐量试验 (OGTT)的血清胰岛素 (INS)和 C肽 (CP)浓度。以腰臀围比 (WHR)作为估计腹型肥胖的指标 ;以胰岛素敏感性指数 (ISI)作为胰岛素抵抗 (IR)指标。结果显示 ,在总体脂相似的情况下 ,EH伴肥胖者腹部脂肪蓄积较多 ,循环胰岛素清除率下降明显。提示 EH伴肥胖患者较单纯性肥胖者有更严重的 IR和高胰岛素血症 (HI)。 EH患者的 IR不但与总体脂增多有关 ,而且与体脂分布关系密切

【Abstract】 The plasma insulin and C peptide concentrations of fasting and oral glucose tolerance test were assayed with RIA method in 21 obese hypertention patients, 19 primary obesities and 16 no obese control subjects. Waist to tip ratio was taken as the index of body fat distribution and insulin sensitivity index (ISI) was used as the index of insulin resistance. The results showed that with similar BMI, obese hypertention patients may have more abdominal fat accumulation than primary obesities. Significantly decrease of the clearance of insulin by liver was also observed in this group. So obese hypertention patients may have more severe insulin resistance and hyperinsulinemia. In obese hypertention patients, Increased total adipose content was related to insulin secretion, while increased intra abdominal adiposity was more significantly correlated with insulin resistance. This suggests that generalized and regional adipose tissue depots are independent factors accounting for insulin resistance in essential hypertention patients.
