【摘要】 目的　了解视频脑电监测与常规脑电图在癫痫诊断中的使用价值。方法　采用国内SW系统智能化脑电图仪 ,对 375例临床确诊为癫痫 (2 93例 )和可疑癫痫 (82例 )患者进行了视频脑电监测 ,并同时进行常规脑电图检测。结果　视频脑电监测异常者 30 7例 (81.9% ) ,其中有痫样放电者 2 2 7例 (73.9% ) ,于清醒时出现者 42例 (18.5 % ) ,于睡眠期出现者 115例 (5 0 .6 % ) ,主要出现在NREMⅠ -Ⅱ期。另有 70例于清醒及睡眠期均出现痫样放电。监测期间出现临床发作 12 0例 (32 % )。 375例同时行常规脑电图检查 ,异常者 15 7例 (4 1.8% ) ,其中有痫样放电者 73例 (4 6 .4% ) ,出现临床发作 8例 (2 .1% )。异常率和痫样放电检出率视频脑电监测均明显高于常规脑电图检查 (P <0 .0 0 5 )。结论　视频脑电监测对癫痫的诊断具有重要意义 ,不仅提高了临床诊断、分型以及鉴别诊断价值 ,而且对癫痫术前评估提供了可靠信息更多还原

【Abstract】 Objectives To evaluate the value of video electrical brain monitor(VEM)and routine electroencephalography(EEG)in diagnosis of epilepsy.Methods Using SW type system intelligent EEG,VEM was carried out in 375 cases with epilepsy,doubtful epilepsy,in compartison with examiantion of routine EEG.Results Abnormal VEM was recorded in 307 patients(81.9%).Two hundred twenty seven of 307 with abnormal VEM had epileptiform discharge,including 42 cases(18.5%) during wakefulness,115 cases(50.6%) during sleep and 70 cases both wakefulness and sleep.During VEM,there were 120 cases with clinical attack(32%).Meanwhile 157 abnormal EEG were shown in 375 patients(41.8%).Seventy-three of 375 patients(46.4%) had epileptiform discharge and 8 patients(2.1%)with clinical attack.The positive rate of VEM was obviously higher than that of EEG in epilepsy(P<0.01).Conclusions the VEM is important value for diagnosis,differential diagnosis and typing of epilepsy.更多还原
