【摘要】 目的 :通过检测bcl- 2 ,c -erbB - 2基因在胃粘膜良恶性病变中的表达 ,探讨其对提高胃癌诊断率的作用。方法 :采用免疫组化SABC法 ,检测bcl- 2 ,c -erbB - 2基因在 98例胃癌 ,40例不典型增生和 2 0例正常胃粘膜中的表达。结果 :a .bcl - 2在正常胃粘膜中有弱阳性表达 (10 0 % ) ,在不典型增生中为 5 0 % ,在胃癌中为44 9% ,不典型增生与胃癌中的表达均显著高于正常胃粘膜 (P <0 0 5 ) ,bcl- 2表达与胃癌的分化程度有关 (P <0 0 5 ) ,低分化者bcl- 2表达率高 ,与淋巴结转移 ,浸润深度无关 (P >0 0 5 )。b .c -erbB - 2在胃癌中的阳性表达率为 5 6 1% ,其中在早期胃癌中的表达低 (16 7% ) ,在重度不典型增生和进展期胃癌中的表达高 ,分别为 80 % ,5 8 7% ,二者与早期胃癌比较有统计学意义 (P <0 0 5 ) ,c -erbB - 2表达与浸润深度 ,淋巴结转移有关 (P <0 0 5 ) ,与分化程度无关 (P >0 0 5 )。结论 :bcl - 2 ,c -erbB - 2参与了胃粘膜的癌变过程 ,联合检测bcl - 2 ,c -erbB - 2可能作为胃癌高危人群筛选的有效指标。更多还原

【Abstract】 Objective:To detect the expression of bcl-2 and c-erbB-2 in gastric mucosa of benign and malignant lesions and estimate its diagnosis value. Methods:Using S-P method. Results:a. The positive rate of bcl-2 expression was 10%, 50% and 44.9% in normal gastric mucosa, atypical hyperplasia and gastric cancer respectively. Significant difference was seen between the two groups and normal gastric mucosa(P<0 05). There was relationship between bcl-2 expression and differentiation of gastric cancer, but bcl-2 expression was not related with lymphnode metastasis(LNM) and depth of infiltration. b. The expression of c-erbB-2 gene was found in gastric cancer (56.1%) and low in early stage(16.7%), but high in the advanced stage(58.7%), the heavy atypical hyperplasia(80%), there was a significant difference between the two groups and the early stage of gastric cancer (P<0 05). The expression of c-erbB-2 had something to do with LNM depth of infiltration(P<0 05) and nothing to do with differentiation(P>0 05). Conclusion:bcl-2 and c-erbB-2 may be involved in the course of stomach carcinogenesis, detecting the expression of bcl-2 and c-erbB-2 may be a useful marker for identifying patients at high risk of developing gastric cancer.更多还原
