【摘要】 目的:分析胃癌螺旋CT动态双期增强扫描的影像表现,进一步评估螺旋CT对胃癌的检出及分期。方法:对42例术前胃镜证实为胃癌的患者进行双期动态螺旋CT扫描,分析CT图像上肿瘤的胃壁侵犯情况及淋巴结检出情况,并与手术、病理进行对照。结果:CT扫描对早期胃癌的检出率为50%,进展期胃癌的检出率为100%,对T1,T2,T3,T4期的准确性分别为50%,833%,60%,333%。胃癌CT扫描显示胃壁局限性或广泛性增厚,动脉期不均匀强化,静脉期均匀强化。结论:胃壁增厚及动脉期不均匀强化对诊断胃癌有意义。虽然螺旋CT对淋巴结的检出率较低,但此方法对临床的手术或非手术治疗还是有重要的指导意义。因此,我们建议胃癌患者术前行螺旋CT动态双期增强扫描。更多还原

【Abstract】 Objective:To analyze the two-phase dynamic CT features of gastric carcinoma and evaluate its role in staging and detection of gastric carcinoma.Methods:Two-phase dynamic CT was performed in 42 patients with gastric carcinoma before operation.CT images were analyzed for tumor infiltration of gastric wall,and TNM staging criteria were applied according to UICC classification.The results were correlated with histopathologic findings.Results:The detectability by two-phase enhanced CT scanning of early and advanced gastric carcinoma was 50% and 100%,respectively.Its accuracy of T1,T2,T3,T4 was 50%,83 3%,60%,33 3%,respectively.Mural thickening as well as marked contrast enhancement in the arterial-phase of the gastric wall were closely related to gastric carcinoma.Conclusion:The detection rate of the lymph node is somewhat low.Nonetheless,this method is a useful guide for choosing between tumor resection and nonoperative treatment of patients.We therefore recommend the method of choice for preoperative imaging of gastric carcinoma.更多还原
