肝性脑病与一氧化氮关系的初步研究

【摘要】 目的 :研究血浆NO水平与肝性脑病发生的关系。方法 :用镉还原法测定正常对照、无肝性脑病肝硬化组、肝性脑病组血浆NO(NO-2 /NO-3 )的水平 ,并测心率、平均动脉压。结果 :肝性脑病组血浆NO水平 (5 5 .1± 18.6umol/L)比单纯肝硬化组(4 4.1± 2 0 .3umol/L)高 ,两组均比正常对照组 (2 9.6± 11.4umol/L)高 ,肝性脑病组、肝硬化组血浆NO与平均动脉压呈负相关。肝性脑病Ⅳ级患者血浆NO水平显著高于Ⅰ级患者。结论 :NO除与肝硬化的高动力循环有关外 ,尚与肝性脑病的发生、发展有关

【Abstract】 Objective: To study the relationship between changes of plasma NO(NO - 2/NO - 3) and pathogenesis of hepatic encephalopathy (HE). Methods: Changes of NO levels in plasma of cirrhotic patients with HE and without HE were measured by reduction of cadmium column, normal plasma served as controls. Heart rate(HR) and mean arterial pressure (MAP) were also measured. Results: Plasma NO levels were significantly increased in both cirrhotic patients without HE and patients with HE compared with controls(44.1±20.3umol/L; 55.1±18.6umol/L vs. 29.6±11.4umol/L; P <0.01) and NO levels in patients with HE are higher than that without HE( P <0.05).Plasma NO levels in patients with HE Ⅳ grade were higher than that in patients with HEⅠgrade( P <0.05) .Mean arterial pressure was lower in cirrhotic patients with or without HE than in controls(77.8±10.5, 79.5±10.2 vs. 91.8±9.1 mm Hg; P <0.05). NO levels had significant correlations with mean arterial pressure( r=-0.52,-0.49; P <0.01) in patients with or without HE. Conclusion: The activity of the NO pathway was increased in cirrhotic patients with or without HE and might contribute to the hemodynamic alteration. NO also might play a role in the pathogenesis of hepatic encephalopathy.
