口腔鳞癌p16基因的表达及其病理意义

【摘要】 目的 探讨 p16基因在口腔鳞癌中的表达情况及其病理意义。方法 利用原位杂交和 Western blot-ting技术检测了 48例口腔鳞癌标本和 6例正常口腔粘膜标本 p16基因 m RNA的分布和强度及 p16蛋白的表达 ,并分析了 p16蛋白表达和患者临床病理参数的关系。结果  6例正常口腔粘膜 p16 m RNA表达均阳性 ;48例口腔癌标本中有 2 1例 p16 m RNA表达阴性 (4 3.7% ) ,2 6例无 p16蛋白表达 (5 4.2 % ) ,其中 2 1例 m RNA表达阴性的病例均无 p16蛋白表达 ,另外 5例无 p16蛋白表达的鳞癌组织有 p16 m RNA杂交信号 ;p16蛋白表达与肿瘤的部位、癌细胞的分化程度及临床分期均无关 ,但与颈淋巴结是否转移密切相关 (P<0 .0 1)。结论  p16基因产物丧失常发生于口腔鳞癌 ;p16基因可能通过某种机制抑制癌细胞的转移特性。

【Abstract】 Objective To study the expression of p16 gene in oral squamous cell carcinoma and its clinicopathological meaning. Methods 48 patients with oral squamous cell carcinoma and 6 normal oral mucosa were included in this study; In situ hybridization was used to investigate distribution and intensity of p16 mRNA in cancer and normal tissue, expression of p16 was detected by Western blotting; The relationship between p16 protein expression and location, differentiation and clinical stage was analyzed using x 2 test. Results p16 mRNA expression was all positive in 6 normal mucous tissue; 21 of 48 (43.7%) cases with oral squamous cell carcinoma were found to have negative expression of p16 mRNA, the hybridization signal located in plasm of cancer cells; Western blotting showed that 26 cases had no p16 protein, 21 patients with no mRNA signal were all found no p16 protein expression, however, the rest 5 cases without p16 protein could be seen to have mRNA hybridization signal in cancer cells; Statistical analysis showed that p16 protein expression had no correlation with location, differentiation and clinical stage of oral squamous cell carcinoma, but significant relationship was found between p16 protein expression and metastases of cancer to neck lymph nodes ( P <0.01). Conclusion Lose of p16 gene product is a common event in development of oral squamous cell carcinoma and probably affect the metastatic characters of cancer cells.
