【摘要】 目的 :探讨干扰素联合苦参素治疗慢性乙型肝炎抗病毒及抗纤维化的疗效。方法 :98例慢性乙型肝炎 ,随机分为两组 ,治疗组 5 1例 ,对照组 47例 ,治疗组采用干扰素α - 2a(IFNα 2a) 5 0 0万u/d ,连用 15d后改隔日 1次 ,疗程 6个月 ,同时加用苦参素注射液 40 0mg/d ,疗程 3个月。对照组单用IFNα 2a ,剂量疗程相同 ,在疗程结束时 ,观察两组患者肝功能、乙型肝炎病毒 (HBV)标志及血清肝纤维化等指标变化。结果 :干扰素联合苦参素治疗组患者丙氨酸转氨酶 (ALT)复常率为 82 .3% ,两组比较差异有显著性 (χ2 =4.310 ,P <0 .0 5 )、HBeAg阴转率分别为 5 4.9%与 34 % ,HBVDNA阴转率分别为 5 8.8%与 40 .4%。治疗组血清透明质酸 (HA)、Ⅲ型前胶原 (PCⅢ )、Ⅳ型胶原 (Ⅳ C)等指标与单用干扰素对照组比较差异有显著性 (P <0 .0 5 )。结论 :α 干扰素联合苦参素可能具有抑制病毒复制 ,改善肝功能和抗肝纤维化等功效 ,建议作为慢性乙型肝炎的一种治疗方案 ,进一步验证。更多还原

【Abstract】 Objective:To assess the clinical efficacy of interferon α 2a combined with oxymatrine in the treatment of chronic hepatitis B and in the prevention of hepatic cirrhosis. Methods: 98 patients with chronic hepatitis B were randomly allocated into two groups. 51patients in treatment group were given interferon α-2a 5×10 6?u/d im×15?d, then changed to 5×10 6?u/d im qod up to 6 months; combined with kushensu 400 mg im qod for three months. 47 patients in control group were given the same dose of interferonα 2a as above only. After completion of therapy, liver function tests, markers of hepatitis B virus and liver fibrosis of the two groups of patients were compared. Results: In therapeutic group, serum ALT decrease to normal range in 82.3% of the patients which were significantly different from that of control group ( χ 2=4.310, P <0.05). HbeAg turned to negative in 54.9% of patients in treatment group compared with 34% in control group; HBV DNA turned to negative in 58.8% and 40.4% of patients in the two groups respectively. Levels of serum hyaluronidase(HA), pro collegen type Ⅲ(PCⅢ) and collegen type Ⅳ(Ⅳ C) were significantly different between the two groups( P <0.05). Conclusions: Interferon α 2a combined with oxymatrine may be able to inhibit HBV replication, improve liver function and prevent hepatic fibrosis. It is suggested to be further evaluated in the treatment of patients with chronic hepatitis B.更多还原
