【摘要】 目的探讨结肠癌急性肠梗阻的外科处理。方法回顾性分析1995年1月至2000年12月收治的75例结肠癌急性梗阻的外科诊断和治疗资料。结果75例病人,I期切除吻合51例,其中行右半结肠切除20例,横结肠切除9例,左半结肠切除12例,乙状结肠切除10例。有19例左半结肠癌和乙状结肠癌行I期切除,近端结肠造瘘,II期闭瘘手术。施行I期结肠造瘘,II期肿瘤切除肠吻合,左半结肠癌1例,乙状结肠癌2例。肿瘤无法切除行盲肠造瘘和乙状结肠造瘘各1例。术后吻合口漏发生率2.66%(2/75)。结论充分认识结肠癌急性梗阻是晚期结肠癌的表现,及时明确诊断,尽早手术,根据病人全身状况及术中情况,合理选择术式,做好围手术期处理,可以减少并发症,提高治疗效果。更多还原

【Abstract】 Objective Investigation of the diagnosis and surgical treatment of coute intestine obstruction induced by colon cancer.Methods75cases of canceris intestine obstruction patients admitted dur-ing1995.1to2000.12were investigated retro-spectively.Results Of 75cases,51cases were reseted and arastomosed in period I,among them20cases were of right half colon cancer,9cases of transverse colon cancer,12cases of left colon cancer,10cases sigmoid colon cancer.19cases of left colon cancer had one stage resection operations with the proximal segment ostomy .with one case of left colon and two cases of sigmoid colon cancer had ostomy first then had the radical operation lat-er.For unresectabely cases had cecum or sigmoid colon obtomy respectively.The leakage of stoma only happened in those one stage radical resection and anstomosis of the left colon cancer patients.The rate of leakage is obout 2.66%(2/75).Con-clusion We must notice that the patients of colon cancer with acute intestine obstruction are belong-ing to the late stage of canar.We must also diag-nosis and rave operation promptly and early.It is important to select suitably operative way accord-ing to patients status and the findings during oper-ation.To hand properly befor and after operation can reduce complication and raise the effect of treatment.更多还原
