【摘要】 目的探讨韧带转移、张力带内固定治疗重度锁关节脱位的疗效。方法1995年6月～1999年7月对26例肩锁关节脱位,关节间取间隙>5mm患者,行切开探查术见:喙锁韧带均完全断裂、回缩。复位脱位之肩锁关节由肩峰外侧经肩锁关节面向锁骨中外1/3平行钻入两枚克氏针,并以张力带钢丝固定。缘肩韧带转移、重建喙销韧带。1a后取出内固定。结果26例患者平均随访2a(18～35个月),未发现张力带松力、克氏针退出:对比内固定取出前、后的X线片,肩关节脱位无复发;关节主、被动活动范围正常;2例肩部偶有轻度疼痛。结论对于重度肩销关节脱位,张力带固定确实,喙肩韧带转移重建喙锁韧带操作简便、创伤小。术后关节功能恢复好,两者结合使用不失为一种思想的治疗方法。更多还原

【Abstract】 Objcective To explore the clinical out-comes of using ligament transfer and tension band internal fixation to treat severe dislocation of acromioclavicular joint.Methods Between June1995and July1999,26patients who suffered from severe dislocation of acromioclavicular joints,whose intervals of joints were more than5mm,were found the coracoclavicular ligaments were total broken during operation.After reduction fixation was achieved using two kirschner wires through the acromioclavicular joint to react distal clavicle in combination with a tension hand wire.Coracoacro-mial ligaments were transferred to reconstruct coracoclavicular ligaments.The internal fixation were taken out postoperation1year.Results26pa-tients were evaluated at a mean of2years(ranging from18to35months).The tension bands hadn`t been loose.Comparing the x-ray tomorgram on which there were the internal fixation with the x-ray tomogram without the internal fixation.We found the acromioclavicular didn’ t obtain disloca-tion again.The active scope of shoulder joint were normal.2patients suffered from shoulder pain.Con clusion The tension band fixing the severe dislocation of acromioclavicular joint were stable.The operation of transfering coracoacromial ligament to reconstruct coraloclaricularligament was conve-nient and no one developed motor deficit.Combin-ing both of them for treating severe dislocation of acromioclavicular joint is an excellent method.更多还原
