颅中窝底巨大海绵状血管瘤的诊断和治疗

【摘要】 目的 回顾临床表现和影像学特征及不同治疗方法的结果,探讨颅中窝巨大海绵状血管瘤(GCH)诊断和治疗方法。方法9例颅中窝巨大海绵状血管瘤,行额颞开颅去骨瓣减压术,全切除4例,部分切除1例,活检4例,活检中2例及部分切除1例行放射治疗。结果 无手术死亡。随访期9月~11.5年,4例全切除者无肿瘤复发,3例放疗者肿瘤消失,未治疗者2例无变化。结论 CT、MRI和CAG对诊断GCH有重要价值,对肿瘤放疗和人工栓塞可以增加手术全切除率。放疗对巨大海绵状血管瘤有效。

【Abstract】 Objective To research the diagnosis and treatment of giant cavernous hemangioma (GCH) in middle cranial fossa. Methods 9 cases of middle fossa GCH were operated with frontotempo-ral craniotomy and removal osteoflap depressure. Among them, 4 cases were tatal resected, 1 case was partial resected, 4 cases were biopsied. Radiotherapy was done in one partial resection case and 2 cases of 4 biopsy patients. Results 9 months- 11.5 years following - up, there were no mortality. There were no recurrence in 4 cases total removal, disappeared in 3 radiotherapy cases and no changes in 2 untreat-ment cases after biopsy. Conclusions CT. MRI and CAG are useful in the diagnose of GCH, an assistant radiotherapy and an artificial embolism may increase the rate of tumor removal.
