【摘要】 目的评估曲马多应用于全身麻醉的临床效果。方法选择ASAI～Ⅱ级子宫手术患者 6 0例 ,采用曲马多 4mg/kg(T组 ,n =30 )或芬太尼 4μg/kg(F组 ,n =30 ) ,丙泊酚 2 5mg/kg和维库溴铵 0 1mg/kg诱导插管 ,术中以维库溴铵、丙泊酚 (0 1～ 0 2 )mg·kg-1·min-1持续静滴 ,曲马多 (5 0～ 10 0 )mg/h或芬太尼 (0 0 5～ 0 1)mg/h维持麻醉。监测麻醉前、插管前后、切皮前及之后 5、15、30、6 0、90min的MAP和HR ,拔除气管导管后呼吸空气5、10、15、30min的SpO2 ,评估麻醉满意度。结果术中麻醉效果均满意。T组在诱导插管时MAP改变不明显 ,而F组明显下降 (P <0 0 0 1) ,两组间比较差异有显著性意义 (P <0 0 5 )。术毕拔管后 15min内呼吸空气时SpO2 值T组明显高于F组 (P <0 0 5 )。结论曲马多应用于全身麻醉效果确切 ,而且麻醉诱导时血流动力学改变不明显 ,术后呼吸抑制程度较轻。更多还原

【Abstract】 Objective This paper is to valuate the clinical effect of tramadol versus fentanyl on general anesthesia.Method 60 uterine surgical patients were randomly divided into two groups.Anesthesia was induced with either 4mg·kg -1 tramadol(group T,n=30)or 4μg·kg -1 fentanyl (group F,n=30),2.5mg·kg -1 propofol,0.1mg·kg -1 vecuronium,and maintained with intravenous drips of vecuronium,0.1～0.2mg·kg -1 ·min -1 propofol and either tramadol 50～100mg per hour or fentanyl 50～100μg per hour during operation.MAP and HR were measured on the time of before anesthesia,before and after induction,before incision and 5,15,30,60,90min after incision.SpO 2 was detected on the time of 5,10,15,30 min after extubation.The effects of anesthesia were valuated.Result The effects of anesthesia of all patients were satisfactory.MAP in group F decreased significantly(P<0 01),however,that in group T did not change.There was significant difference between these two groups (P<0 05). The values of SpO 2 in group T were much higher than those in group F (P<0 05) within 15min after extubation.Conclusion Not only the effect of tramadol on general anesthesia was satisfactory,but also the effects of tramadol on hemodynamics and post operative respiratory inhibition were insignificant.更多还原
