【摘要】 目的 :探讨老年性不典型心梗的临床特点 ,提高临床诊断水平。方法 :回顾性分析近 10年来笔者所收治的 16例不典型急性心梗患者的临床资料。结果 :男 11例 ,女 5例 ,平均年龄 65岁。其中心内膜下急性心梗 6例 ,急性下壁心梗 2例。急性下壁心梗并前壁心梗 1例 ,急性正后壁心梗3例 ,前间壁心梗 2例 ,前壁心梗 2例。误诊时间为 6小时至 7天。易误诊为急性心衰、急腹症、肺心病、心绞痛、急性心律失常、急性胆囊炎胆石症、牙痛及其它疾病。结论 :目前急性心梗发病率呈上升趋势 ,临床医生要重视不典型急性心梗的特殊表现 ,动态观测心电图及酶学改变 ,提高诊断水平 ,降低病死率更多还原

【Abstract】 Objective:To evaluate the clinical characteristics of senile atypical myocardial infarction and to raise the clinical diagnosis level.Methods:Sixteen Cases of atypical acute mycocardiac infarction treated by the writer in the recent 10 years were retrospectively analyzed.Results:The ratio of male to female was 11∶5 in 16 patients (11 males,5 females).The mean age was 65 years.Among these patients,acute subendocardidal infarction was diagnosed in 6 cases,acute inferior myocardial infarction in 2 cases,acute inform and anterior myocardial infarction in 1 case, acute positive posterior myocardial infarction in 3 case,anteroseptal myocardial infarction in 2 cases,anterior myocardial infarction in 2 cases.The misdiagnosis time was from 6 hours to 7 days.They were misdiagnosed easily for acute heart failure,acute abdomen,pulmonary heart disease,angina pectoris,acute arrhythmia,acute cholecystitis and cholelithiasis,toothache and other diseases.Conclusion:At present,the disease incidence of acute myocardial infarction is trending upwards.For raising the diagnosis level and decreasing the death rate,the clinician should pay attention to the special presents of atypical allot myocardial infarction,observe the dynamic electrocardiogram and the changes of zymology.更多还原
