【摘要】 目的　探讨颈椎脊髓损伤围手术期的呼吸管理 ,为提高临床疗效提供有效的保障。方法　本组共有 12 6例颈椎脊髓损伤病人 ,其中男 94例 ,女 32例 ,颈椎骨折脱位 84例、颈椎过伸伤 30例、急性颈椎间盘突出症 12例 ,年龄 18～ 78岁 ,平均40 . 2岁。前路手术治疗 98例 ,后路手术 10例 ,前后路联合手术 18例。气管切开 8例 ,气管插管机械呼吸 6例 ,合并呼吸道感染 2 6例。结果　随访半个月～ 10个月 ,呼吸功能完全恢复者 12 4例 ,2例死亡。结论　颈椎脊髓损伤围手术期呼吸管理是颈椎手术疗效的关键因素 ,只有积极和合理的管理 ,才能为临床治愈提供保障更多还原

【Abstract】 Objective To investigate the respiratory control over the patients with cervical spinal cords injuries so as to improve clinical outcomes.Methods There are 126 cases in this group, of which 94 cases were male, 32 cases female. Eighty four patients with cervical fracture and dislocation,30 patients with extension injuries in the cervical spine, 12 patients with acute cervical disc herniation. The average age was 40.2 years old with a range from 18 to 78 year old.Ninety eight cases were operated through anterior approach, 10 cases through posterior approach, 18 cases through combined approach. Tracheotomies were done in 8 patients, trachea intubation with assistant respiration in 6 patients.Twenty fix cases were assoiated with respiratory tract infection.Results The respiratory function of 124 patients recovered completely and 2 cases died during the follow up between 0.5 months and 10 months postoperatively.Conclusion The perioperative respiratory control over the patients with cervical spinal cord injuries plays pivotal role in the surgical outcomes of the cervical operations. Only with active, appropriate managment can the patients achieve optimistic surgical outcomes.更多还原
