【摘要】 目的　研究A组乙型溶血性链球菌菌膜抗原在淋巴细胞贴壁抑制试验中诊断风湿性心脏炎的特异性。方法　用A组乙型溶血性链球菌菌膜抗原和 1%植物血凝素 (PHA)作为刺激物 ,分别测定 38例健康对照组 ,30例风湿性心脏病活动组 ,2 4例风湿性心脏病静止组外周血淋巴细胞贴壁抑制百分率 (LAIP)。结果　在特异性菌膜抗原刺激下 ,风心活动组LAIP为 ( 34.47± 4.5 6 ) % ,显著高于风心静止组 ( 12 .5 0± 3 .73) %和对照组 ( 13 .5 6± 3 .31) % ,而后二者差异无统计学意义。在受PHA刺激后 ,风心活动组LAIP为 ( 8.95± 2 .5 2 ) % ,风心静止组 ( 6 .2 3± 1.6 1) % ,对照组 ( 7.5 1±3 16 ) % ,三者差异无显著性。结论　以链球菌菌膜为促凝刺激物的LAIP试验在判断风湿性心脏炎方面具有特异性 ,其特异性取决于所用的刺激原为特异性抗原更多还原

【Abstract】 Objective To study the specificity of antigen from group A streptococcal cell membrane in procoagulant activity assay for the diagnosis of rheumatic carditis. Methods A specific antigen from group A streptococcal cell membrane and 1% PHA were used as stimulant to test the peripheral blood leucocyte adherence inhibition percent (LAIP) in 30 cases of active rheumatic heart diseases, 20 cases of inactive rheumatic heart diseases and 38 cases of normal controls. Results When streptococal cell membrane was used as a stimulant, LAIP in active rheumatic heart diseases group(34.47±4.56)% was significantly higher than that in inactive rheumatic heart diseases group(13.56±3.31)% and in normal control group(12.50±3.73)%. No statistical difference was found between the latter 2 groups ( P >0.05); when 1% PHA was used as stimulant ,there was no statistical difference among all the three groups ( P >0.05), LAIP in active rheumatic heart diseases, inactive rheumatic heart diseases and in normal control group was (8.95±2.52)%,(6.23±1.61)%,(7.5±3.16)% respectively. Conclusion\ LAIP is of specific significance for detecting the activity of rheumatic carditis and its specificity is based on the specific antigen from group A streptococcal membrane.更多还原
