【摘要】 目的 :介绍应用神经内镜清除脑内血肿的经验。方法 :应用神经内镜清除 15例脑内血肿 ,总结手术指征、手术要点和疗效。选择病例具有以下特点 :病因为外伤或高血压 ;手术时间为发病后 5～ 36h ;无脑疝、深昏迷及脑中线结构移位 >1.0cm者。手术要点 :术前精确定位 ;熟悉镜像特征 ;手术原则为最大限度地清除血肿和减少出血。结果 :治愈 14例 ,1例术前即出现肾功能衰竭而死亡 ,全组未出现新的神经功能障碍 ,6例偏瘫有所恢复。结论 :应用神经内镜清除脑内血肿是一种安全、有效的微创手术 ,其要求是选择合适的病例和掌握神经内镜的操作方法及清除脑内血肿的基本原则更多还原

【Abstract】 Objective:To introduce the method of evacuating intracerebral hematomas with neuroendoscope.Methods:Fifteen cases of intracerebral hematoma were treated by neuroendoscopic surgery and the indications,effect and main techniques of the operation were analyzed.These selected patients had the following mutual characteristics:(1)the cause of intracerebral hematoma was trauma or hypertension;(2)the operation was performed 5 to 36 hours after the disease occurred;(3)no deep coma or brain hernia occurred and the middle structure of intracrany did not move or move very little(less than 1 cm).The key to the operation was as follows:(1)each lesion was precisely located prior to operation;(2)the surgeon knew the features of lesion imagines and neuroendoscopy very well;(3)the operative principle was to evacuate hematomas and avoid bleeding as far as possible.Results:After the surgery,14 cases were cured,one died of renal function failure.Conclusions:As a minimally invasive technique,neuroendoscopy is efficient and safe for the evacuation of intracerebral hemaomas.However,it is very important to strictly adyhere to the proper indications and basical operative principles.更多还原
