三棱内固定器的研制与临床应用

【摘要】 为探讨三棱内固定器治疗股骨颈骨折的疗效 ,采用摹拟股骨颈骨折三棱内固定器固定的方法 ,对自行设计的三棱内固定器进行了生物力学性能测试。结果显示 ,它能提高固定能力 ,制约钉的滑动和骨折端的旋转 ,减少骨折端的剪切力 ,从而提高了骨折的愈合率和降低股骨头的坏死率。经 30例临床观察骨折愈合率 96 .7% ,功能恢复优良率为 93.3% ,股骨头坏死率为 6 .7%。三棱内固定器利用主钉与固定钉连成一个密闭的钢架固定系统 ,同时根据三角形的稳定性原则将 3枚三角针呈等边三角形排列固定两骨折端 ,固定牢固可靠 ,不易滑脱和旋转 ,是一种良好的治疗股骨颈骨折的方法。

【Abstract】 To explore the therapeutic effect of prismatic internal fixator (PIF) on femoral neck fracture, the biomechanical properties of the self designedPIF were assayed with a method simulating PIF fixation of femoral neck fracture. The results showed that PIF functions to improve the fixating ability, to limit the nail sliding and the fragment rotation, to decrease the fragment shear, thus to improve the fracture healing rate and to lower the femoral head necrosis incidence. The clinical observation of 30 cases treated by PIF showed that the fracture healing rate reached 96.7 %, the good excellent rate of function recovery reached 93.3 %, and the incidence of femoral head necrosis was 6.7 %. PIF uses the main and fixating nails to form a closed steel frame fixating system and the three prismatic nails ranged in an equi lateral triangle to fix the two fragments, thereby has a high fixation rigidity and reliability but unallows the fragments to slide and rotate and is a good method for treating femoral neck fracture.
