复方乳酸钠山梨醇对手术患者血电解质及血糖的影响

【摘要】 目的 :观察复方乳酸钠山梨醇注射液对外科手术患者的血电解质及血糖的影响。方法 :随机选 30例手术患者 ,其中 15例血糖正常为第 1组 ,15例血糖高于正常为第 2组。两组患者于术后d1均输入复方乳酸钠山梨醇 10 0 0ml,d2输入 5 %葡萄糖盐水 10 0 0ml(第 1组 )或 5 %葡萄糖盐水 10 0 0ml加正规胰岛素 10~ 12u(1∶4~ 1∶5 )(第 2组 ) ,在输液前后抽血查血电解质及血糖和胰岛素的值。结果 :第 1组患者输入两种液体后血糖均升高 ,两者升高程度比较无明显差异 (P >0 .0 5 ) ;血电解质较输液前无明显变化。第 2组患者输入两种液体后血糖和胰岛素升高程度比较有明显差异 (P <0 .0 1)。结论 :复方乳酸钠山梨醇是细胞外液良好替代剂 ,可作为手术患者特别是葡萄糖利用障碍 (糖尿病 )患者的供能物质 ,其效果较传统的葡萄糖加胰岛素更好。

【Abstract】 OBJECTIVE:The effect of compound intravenous infusion of sodium lactate and sorbitol (SLS infusion) on blood electrolyte and glucose in surgical patients were studied. METHODS: 30 surgical patinets were enrolled into the study. 15 of them with normal blood glucose (BG) comoposed of group 1 and the balance of them with higher BG composed of group 2. Patients in both groups were treated with 1 000 ml SLS infusion at the first day after operation. In the next day, patients in group 1 were treated with 1 000ml 5% GNS and patients in group 2 were treated with 1 000 ml 5% GNS plus 10~12u regular insulin. Blood level of electrolytes, glucose and insulin was measured before and afert infusions. RESULTS: Bolld glucose level was rose in group 1 patients after both SLS infusion and 5% GNS treatment but no difference ( P> 0.05)in levels of BR increase and electrolytes were observed between the two agents. The levels of BG and insulin in group 2 patients have significant difference( P <0.01)between pre and post treatment with SLS infusioin and 5% GNS plus insulin.CONCLUSION:SLS infusion is a promise substitute for extracellular fluid and can be used as a dynamophore in surgical patients ,especially patients wity diabetes, with an effect better than those of GNS plus insulin.
