单眼弱视患者双眼相互作用的空间特性研究
Spatial selectivity of binocular interaction of monocular amblyopes studied with dichoptic VEP
【摘要】 目的 :研究单眼弱视患者双眼相互作用 (binocularinteraction ,BI)的空间特性 ,探讨其视觉传入系统的病理生理改变 ,为客观检查双眼视功能提供依据。方法 :利用同视机分视双眼 ,用计算机产生 5种黑 白棋盘格翻转刺激 ,依次记录正常及单眼弱视患者各 30例的稳态视觉诱发电位 (visualevokedpotential,VEP) ,进行Fourier分析 ,通过主导眼或健眼在双、单眼状态下的能量之比 (B/M)来评价BI。结果 :①当双眼棋盘格大小(checksize ,CS)相同时正常人的B/M最小 ,随CS差异的增加B/M逐渐增大。②单眼弱视者当双眼CS相同时B/M最小 ,但较正常人明显增大。结论 :BI存在空间特性 ,即双眼CS相同时BI最大 ,随CS差异的增大BI逐渐减小。而单眼弱视者BI明显减小 ,其处理空间信息的能力减弱。提出双眼分视下CS为 15′~ 12 0′ ,特别是30′时可以作为单眼弱视病人双眼分视VEP检查的理想刺激条件
【Abstract】 Objective:The aim of the present study was to determine the spatial selectivity of binocular interaction in monocular amblyopia, to discover the physiopathologic changes of visual system in amblyopia.Methods:Steady state VEPs elicited by dichoptic reversal checkerboards with check sizes ranging from 10~120 min of arc were recorded on 30 monocular amblyopes and 30 normal subjects. The binocular interaction was measured by the Binocular/monocular VEP amplitude ratio (B/M) in the normal eye of amblyopes or the dominated eye of normal subjects.Results:①In normal subjects, when each eye was stimulated by the same size, the B/M was minimum. The value of B/M increased as the check sizes before each eye were made progressively more unequal. ②In cases of monocular amblyopia the B/M reached its minimum value when each eye was stimulated by the same check size, but it was statistically larger than the normal value.Conclusion:Binocular interaction showed spatial selectivity to pattern stimulus in normal subjects. It was most pronounced when the checkerboard sizes of the stimulus before each eye were identical and the interaction decreased with increasing difference between the sizes of both eyes. In patients with monocular amblyopia, the binocular interaction was obviously decreased or even disappeared, and the ability of processing spatial information decreased. The checkboard sizes 15′~120′ especially 30′ is the ideal stimulus in dichoptic steady state VEP examination for monocular amblyopia. [FK(WB80011。6]
