角膜塑型术后角膜地形图早期改变
The early evaluation of corneal topography on Orthokeratology
【摘要】 目的 :观察近视和近视散光患者进行角膜塑型术 (Orthokeratology,简称OK镜 )治疗前后的角膜地形图早期改变 ,有效预测OK镜的治疗效果和指导验配。方法 :对使用OK镜的近视和近视散光患者 ,共 2 6例 5 0只眼 ,在治疗前和治疗后 1天、一周、二周、三周、四周、二个月做屈光状态检查和计算机辅助的角膜地形图检查。患者按等值球镜度分为两组 ,Ⅰ组屈光度 <- 5 .0 0D共 15例 (2 9只眼 ) ,治疗 1个月以上者 2 9只眼 ,2个月者 18只眼 ,Ⅱ组屈光度≥ - 5 .0 0D共 11例 (2 1只眼 ) ,治疗 1个月以上者 2 1只眼 ,2个月者 11只眼。结果 :治疗后角膜曲率达最佳治疗效果所需时间为 :Ⅰ组 16 .2± 4.6天 ,Ⅱ组 15 .3± 4.1天 ,分别对应屈光度改变 3.3± 0 .9D及 4.6± 1.0D。治疗前地形图形态 :Ⅰ组中圆形 31.0 % ,蝶形 6 9.0 % ,不规则形 0 % ,Ⅱ组中圆形 9.5% ,蝶形 80 .9% ,不规则 9.5 % ;治疗后Ⅰ组中环形 37.9% ,半环形 5 8.6 % ,蝶形 0 % ,中心岛形 3.4% ,Ⅱ组中环形 47.6 % ,半环形 47.6 % ,蝶形 4.8% ,中心岛形 0 %。两组治疗前后的角膜规则指数 (SRI)、角膜非对称指数(SAI)以及角膜平坦K值差异存在显著性 (P <0 .0 0 1,P <0 .0 0 1和P <0 .0 1)。两组的角膜陡峭K值在治疗前后差异无显著性。结论 :计算机辅助的
【Abstract】 Objective: To study the early changes of corneal topography on Ortho-K patients before and after the therapy, makes effective prediction in the process of fitting. Methods: The study comprised two groups 50 eyes of myopia with or without astigmatism(group 1<-5.00D, group 2≥-5.00D). Computerized corneal topography, ocular health and refraction were examined prior to fitting and then at 1day, 1week, 2weeks, 3 weeks, 4 weeks, and 8 weeks. Results: The maximum decrease of myopia occur at about two weeks, which corresponding with flattening of K. The Surface Regular Index (SRI), Surface Asymmetry Index (SAI) and Flat K showed significant changes (P<0.001, P<0.001 and P<0.01). Conclusion: Computer-aid corneal topography is the most important exam in Ortho-K follow-up, it can’t be replaced by other methods. The change of lens index may refer to the corneal shape and refraction result. [FK(WB80011。5]
