PTCA对血小板Ⅱb/Ⅲa受体及一氧化氮水平的影响及临床意义

【摘要】 目的了解PTCA(经皮冠状动脉腔内成形术)对GPⅡb/Ⅲa(血小板膜糖蛋白Ⅱb/Ⅲa受体)、NO(一氧化氮)水平的影响 ,为临床上减少PTCA术后副作用提供帮助。方法静脉采血 ,采用放免和分光光度比色法检测PTCA术前后GPⅡb/Ⅲa及血浆NO水平。结果PTCA手术后GPⅡb/Ⅲa水平明显增高 ,术后12h达到高峰 ,48h接近术前水平。血浆NO变化与GPⅡb/Ⅲa的变化趋势相反。结论PTCA可能通过增加GPⅡb/Ⅲa表达 ,降低NO合成释放等环节 ,可诱发新血栓形成 ,导致急性冠状动脉闭塞。因此 ,为减少PTCA后急性再梗塞的发生 ,临床上除应用常规的抗凝治疗外 ,加用GPⅡb/Ⅲa受体拮抗剂效果会更好

【Abstract】 Objective To observe the effect of PTCA on platelet GP and NO. To provide reference to reduce side effect after PTCA. Methods Spectrophotocolorimetry and radioimmunoassay were used for the determination of plasma NO and platelet GP levels, respectively.Result The results showed that the levels of platelet GP were higher and plasma NO was lower after PTCA.The level of platelet GP reached peak at 12h and returned low level at 48h. The change of plasma NO was consistent with that of platelet GP. Conclusion PTCA can stimulate expression platelet GP and reduce synthesizing and releasing of NO. These may cause to form fresh thrombus and acute coronary artery occlusion. In order to reduce acute reinfarction, platelet GP integrin antagonists should be used besides clinical routine anticoagulant.
