晚期胰腺癌患者凝血、抗凝及纤溶功能的研究

【摘要】 目的 :研究晚期胰腺癌患者凝血、抗凝、纤溶功能的变化 ,并探讨其临床意义。方法 :以 PT、APTT、Fg、VWF等四项指标检测凝血功能 ,以 AT- A、AT- Ag、PC∶ A反映抗凝功能 ,PL g、tp A∶ A、PAI∶ A、FDP等四项指标反映纤溶功能 ,并与正常人群对照。结果 :在晚期胰腺癌组 ,PT、APTT明显延长 (P<0 .0 1) ,Fg、VWF升高 (P<0 .0 1) ,AT- A%降低 (P<0 .0 1) ,AT- Ag、PC∶ A升高 (P<0 .0 1) ,而 PL g、tp A∶ A、PAI∶ A均降低 (P<0 .0 1) ,FDP明显升高 (P<0 .0 0 1)。结论 :晚期胰腺癌患者存在明显的凝血、抗凝、纤溶异常 ,原因与胰腺癌本身有关 ,如不加以正确处理 ,将使病情恶化。

【Abstract】 Objective:To study the function change of plasma coagulation, anticoagulation and fibrinolysis systems in carcinoma of the pancreas.Methods:22 patients with carcinoma of the pancreas were studied. The parameters of plasma coagulation include PT,APTT,Fg and VWF,the parameters of plasma anticoagulation incude AT ⅢA,AT ⅢAg,PC∶A,the parameters for plasma fibrinolysis were PLg,tPA∶A,PAI∶A and FDP.All these parameters were expressed as ±s .and students t test was used when compared with normal.Results:It was founded that the level of PT,APTT,Fg,VWF were all higher than normal (P<0.01) .AT ⅢA was lower than normal (P<0.01) ,but AT ⅢAg,PC∶A were higher than normal (P<0.01) .Except for FDP,the level of PIg,tpA∶A and PAI∶A were lower than normal (P<0.01) ,but FDP was higher than normal (P<0.001) .Conclusion:In patients with carcinoma of pancreas there were obvious abnormal in plasma coagulation,anticoagulation and fibrinolysis systems.Most patients were in prothrombin status .This prothrombin status is the complication of carcinoma of pancreas itself,and can aggravate if correct therapy was not followed.
