等速肌力测试训练仪量化评定肌痉挛
Quantitative Assessment of Spasticity by Peridulum Test Using Cybex Ⅱ Isokinetic Dynamometer
【摘要】 根据摆动试验原理,利用等速肌力测试训练仪测试下肢股四头肌痉挛的截瘫患者18例和偏瘫患者16例(痉挛组),并测试正常人10例(对照组)作为对照,进行肌痉挛量化评定,分别记录其摆动曲线,以摆动曲线第一波的幅度(A1)、摆动次数(F)、摆动时间(T)、放松指数(R1)、幅度比(R2)为量化指标。结果:痉挛组下肢摆动曲线比较僵硬、不规则,幅度较小、时间缩短、波宽较窄,次数≤3次;对照组则摆动曲线平滑规则,屈伸角度较大,随着摆动次数增加,幅度逐渐减少至停止,摆动次数为4~6次。2组测试数值间比较差异有显著性(P<0.01)。本方法与目前国际公认的Ashworth量表法所测结果间存在良好的相关性。提示:等速肌力测试训练仪效度良好,重测信度高,是一种简单易行又较为实用的量化评定方法。
【Abstract】 Spasticity was quantitatively assessed in 34 spastic patients (including 18 paraplegic patients and 16 hemiplegic patients) and 10 normal control subjects by the peridulum test using Cybex Ⅱ isokinetic dynamometer. Typical pendulum test goniograms of each subject were obtained and 5 parameters, A1 (the flexion angle of the first drop), F (frequency of swings), T (time of swings), RI (relaxation index), R2 (amplitude ratio), were extracted from the goniograms to evaluate the degree of spasticity. The results showed that in spastic group the goniograms of the lower limb were stiff, unregular, of lower amplitude, shortened T, narrower wave width and F≤3. But in the control group, the goniograms were smooth, regular, of larger Al. With the increase of F, amplitude was gradually decreased. F was 4 to 6. There was a significant difference in the tested data between the two groups (P<0. 01 ).The outcomes from this method had a good correlation with those from Ashworth scale method. It was suggested that tbe validity and reliability of this method is high.
