TMR术后激光心肌管道的实验形态学观察

【摘要】 目的 :从形态学角度探讨透室壁性激光心肌血管重建术 (TMR)的供血机理。方法 :采用CO2 激光照射成年兔左室壁 ,形成透室壁性激光心肌管道 ,于术后不同时间处死 ,利用组织学切片染色法 ,观察激光心肌管道内血管的再生与重建及形态学变化。结果 :TMR术后 ,激光心肌管道内相继出现血液凝固、机化、新生血管及纤维组织增生 ,最终由纤维性瘢痕修复。但在纤维性瘢痕中存在着稀少的与心腔相通的小动脉和小静脉。结论 :激光心肌管道本身并不能对心肌直接供血 ,而是由机化过程形成的毛细血管和小血管及纤维瘢痕中存留的小动脉对心肌供血。

【Abstract】 Objective:To study the mechanism of blood supply of transmyocardial revascularization(TMR) from the view of histomorphology.Methods:Under direct vision,transmyocardial channels were made by CO 2 Laster in vivo on the left ventricular wall of 24 rabbits.The animals were sacrificed at intervals of 1,3,7,14,28 and 56 days after TMR.The angiogenesis and histomorphological structure of the channels were observed under microscope.Results:The blood coagulation,granulation tissue,new vessels and proliferation of fibrous tissue appeared in succession in the Laster channels after TMR.The channels were replaced by fibrous tissue eventually.Nevertheless a few small arteries and veins remained in the fibrous tissue of some channels and communicated with the ventricular cavity.Conclusions:The transmyocardial channels itself cannot provide the blood supply to the myocardium.Actually,the capillaries and small blood vessels regenerated in the granulation tissue and the small arteries remained in the fibrous tissue carry out the function of blood supply to the myocardium.
