【摘要】 目的 :评估AF内固定对胸腰段脊柱骨折的疗效及其应用价值。方法 :对 40例患者。术前按Frankel分级评定损伤程度。根据经验选定椎弓根内固定的进钉点 ,行AF内固定。术后以上述分级方法再次对 40例患者进行评定 ,并作X线、CT检查对AF疗效进行分析 ,术后平均随访 1 5个月。结果 :术前Frankel分级B～D级者均获 1～ 2级的进步。全部病例无医源性神经根受损或断钉。X线随访示 3 6例患者椎体序列恢复正常。有 3例术后 6个月后发生钉周脓肿形成。 4例术后椎体前缘高度丢失。CT随访示椎管容积均恢复至正常的 80 %以上。结论 :AF内固定有多重的矫正力 ,能间接复位受伤脊柱 ,恢复腰椎的生理前凸。与其他内固定方法相比较 ,AF系统具有疗效显著、操作简单、固定确切等优点。作者提出的进钉点操作容易、安全性高。更多还原

【Abstract】 Objective:To evaluate the effectiveness and application value of the Atlas Fixation(AF) for thoracolumbar fracture.Methods:Forty patients with fracture of thoracolumbar spine were operated by AF internal fixation.Before operation,the patients were evaluated by the Frankel’s calssification;after operation all patients were evaluated again,including X ray and CT scans.Results:The patients of Frankel’s B～D were improved by 1～2 scroe after operation.There were no iatrogenic nerve root injury and broken screws.Thirty six patients had regained the normal spinal sequence.Of 40 cases ,3 had peri screws abscess and 4 lost the anterior height of spine body.CT scans following up indicated that the spinal canal volumes were all returned to above eighty percent of normal.Conclusion:Compared with other spinal posterior internal fixation,Atlas Fixation has better results in effectiveness and simplification.更多还原
