【摘要】 目的　比较 3种不同长度巩膜隧道外切口超声乳化白内障吸除术后产生手术源性散光(surgicallyinducedastigmatism ,SIA)度数及方向的变化规律 ,并评价手术效果。方法　将 10 4例 ( 10 4只眼 )白内障患者分为A、B、C 3组 ,分别以 3 2、5 5及 7 0mm巩膜隧道外切口施行超声乳化白内障吸除术。测量术前、术后 1周、1、3和 6个月的角膜曲率 ,运用Cravy向量分析法计算SIA度数及方向 ,比较 3组术后不同时间的平均SIA度数及裸眼视力。结果　平均SIA度数和裸眼视力 ,术后 1周A组明显优于B、C组 (P <0 0 5 ) ;术后 1、3和 6个月 3组比较 ,差异无显著性 (P >0 0 5 ) ;术后各随访时间B、C两组比较 ,差异均无显著性 (P >0 0 5 )。结论　以 3 2mm外切口行超声乳化白内障吸除术后SIA度数小而稳定 ,早期可获得良好而稳定的裸眼视力更多还原

【Abstract】 Objective To compare the changes in the values and directions of surgically induced astigmatism (SIA) after phacoemulsification with three differentsized scleral tunnel incisions and to evaluate their clinical results Methods The patients were divided into three groups; the length of the scleral tunnel incision was 32, 55 and 70 mm respectively in A, B and C groups Keratometry measurements were obtained preoperatively, at postoperative 1 week, 1 month and 6 months respectively SIAs were analyzed by “Cravy vector method” The mean SIA and uncorrected visual acuity were compared among the groups up to 6 months postoperatively Results Eyes in A group displayed significantly less mean SIA and better uncorrected visual acuity than B and C groups at 1 week postoperatively (P<005), but there were no significant differences between any groups at 1 month, 3 months and 6 months postoperatively (P>005) There were no significant differences in the mean SIA and uncorrected visual acuity between B and C groups throughout the whole study period (P>005) Conclusions SIAs in eyes of 32 mm incision group of phacoemulsification are less and more stable; a better and quicker rehabilitation of uncorrected visual acuity can be obtained in the early postoperative period更多还原
