胸廓内血管蒂肋软骨瓣移植修复关节软骨缺损的研究
Experiment study on costal cartilage flap for transplantation repairing defects of articular cartilages with internal thoracic blood vessel
【摘要】 目的 为带血供肋软骨移植提供解剖学依据。方法 解剖观测 30侧成人胸廓内血管行程、分支、剑突血供及其形态学。结果 胸廓内动脉沿胸骨旁线下行 ,第 4肋骨之后走行于胸横肌的浅面 ,距正中线 (2 .7± 0 .7)cm ,在第 6肋或第 6肋间隙分为肌膈动脉和腹壁上动脉 ,其分支点距正中线 (2 .8± 0 .5 )cm ,剑突上 (4 .0± 1.3)cm。以上分支沿途分出肋间前动脉的肋软骨支和剑突支。胸廓内动脉外径 (2 .0± 0 .6 )mm、静脉 (2 .2± 0 .9)mm和 (1.5± 0 .8)cm ,剑突支 (0 .9± 0 .1)cm剑突中部长 (2 .1± 2 .3)cm ,中部宽 (1.1± 1.2 )cm ,中部厚 (0 .5± 0 .8)cm。结论 设计以胸廓内血管蒂的第 6肋软骨瓣 (单根 )、第 6、7肋软骨瓣 (双根 )和剑突瓣移植术 3种 ,可用于关节软骨缺损修复。
【Abstract】 Objective In order to provide anatomical basis of transplantation for costal cartilages with blood supply.Methods The course and the branches of the vessels on thorax, the blood supply and the configuration of xiphoid were observed on 30 sides of adult cadaver.Results The internal thoracic artery traveled along by side line of sternum down, to move on the thoracic transverse muscle after 4th rib, to the distance of midmost line was (2.7±0.7) cm, at 6th rib or 5th costal gap sent up musculophrenic artery and superior epigastric artery with the point of sending to midmost line being (2.8± 0.5 ) cm, up the xiphoid being (4.0±1.3) cm. The branches sent along the way up the branch of costal cartilage and the branch of xiphoid at intercostal anterior artery. The internal thoracic arterial exeternal diameter was (2.0±0.6) mm, vein (2.2±0.9) mm and (1.5±0.8) mm, the branch of xiphoid (0.9±0.1) mm. The length, width and thickness of the middle of xiphoid were (2.1±2.3) cm, (1.1±1.2)cm and (0.5±0.8) cm respectively. Conclusion Three transplantation, the bone flap of 6th single costal cartilage and the bone flap of 6th and 7th double costal cartilages, and the bone flap of xiphoid with internal thoracic blood vessel were able to be used to repair defect of articular cartilages.
