乌拉地尔对高血压患者血浆4种神经肽含量的影响

【摘要】 目的 观察乌拉地尔 (URA)对高血压患者血浆 4种神经肽 :内皮素 (ET)、降钙素基因相关肽 (CGRP)、神经肽Y(NPY)及神经降压素 (NT)水平的影响。方法 选择麻醉前高血压患者 2 0例。于麻醉诱导前静脉注射URA 0 .5mg/kg。应用放射免疫方法测定URA治疗前后血浆 4种神经肽的水平 ,并以 2 0例健康者作对照。结果 高血压组血浆ET、NPY水平明显高于正常组 (P <0 0 1) ,血浆CGRP、NT水平明显低于正常组 (P <0 0 1)。静脉注射URA后 5min ,高血压组血浆ET显著性降低且接近对照水平 (P <0 0 1)、NPY较治疗前显著性降低且明显低于对照水平 (P <0 0 1,P<0 0 5 ) ;血浆CGRP较治疗前显著性升高且达对照水平 (P <0 0 1)、NT虽较治疗前显著性升高但仍低于对照水平 (P <0 0 1,P <0 0 5 )。结论 内皮素、降钙素基因相关肽、神经肽Y及神经降压素的比例失衡与高血压的发生密切相关 ,URA可改善其平衡关系 ,可作为其降压机制之一。

【Abstract】 Objective To study the effect of urapidil on the levels of 4 kinds of plasma neuropeptides : endothelin(ET),neuropetid Y(NPY),calcitonin gene-related peptide(CGRP) and neurotensin(NT) in patients with hypertension. Methods Twenty patients with hypertension ,scheduled for surgery,received urapidil 0.5mg/kg intravenously before anesthesia induction .The intravenous blood samples were taken before and 5min after the administration ,to measure the plasma levels of ET,NPY,CGRP and NT with radioimmunoassay . Twenty health humans served as control. Results Compared with control levels, ET and NPY levels increased markedly and CGRP and NT levels decreased significantly in the patients with hypertension (P<0.01). Following urapidil administration , the level of ET decreased significantly and nearly to the control level (P<0.01), NPY level decreased significantly and was lower than the control level (P<0.01, P<0.05), the level of CGRP increased to the control level (P<0.01), the level of NT increased significantly (P<0.01) but was still lower than the control level (P<0.01). Conclusions It may be an important cause of essential hypertension that the proportion of the above plasma neuropeptides levels is out of balance . The improvement of the balances among them may be one of the mechanisms of antihypertensive effect of urapidil.
