多药耐药基因表达及凋亡指数对胃癌预后的影响
Multidrug Resistance Gene Products Expression and Apoptosis Index in Gastric Carcinoma and their Impact on Prognosis
【摘要】 目的探讨胃癌多药耐药(MDR)表达与预后,凋亡指数(AI)与预后及MDR与AI表达之间的关系。[方法]应用免疫 组化的方法及TUNEL法,结合1987年~1991年的完整胃癌病例进行10年随访,对胃癌MDR的主要指标P-gp、GST-π、DNA TOPO Ⅱ及细胞AI进行检测。[结果]P-gp表达与临床分期有关(P<0.05),正常组织表达P-gp高者生存期长(P<0.05)。GST-π表达强度 与预后有关(P<0.05)。TOPO Ⅱ表达与分型有关(P<0.05)。凋亡指数表达与临床分期有关(P<0.05)。P-gp里表达与GST-π表达 之间相关(P<0.05),GST-π与TOPO Ⅱ表达相关(P<0.05),但多药耐药表达与细胞AI之间无关(P>0.05)。[结论]MDR与AI都 属细胞正常基因组表达的一部分,MDR主要功能是细胞防御,其表达产物之间有相关性。细胞凋亡功能为细胞自稳,也由基因控 制,但与MDR调控基因无相关,不同生存年段其MDR与AI表达未见有明显统计差异(P>0.05)。提示单因素分析MDR/AI与预 后尚不够全面。
【Abstract】 Purpose] To study the clinical significance of expression of MDR and Al in gastric cancer. [Methods] Eighty eases of paraffin embedded gastric cancer tissues were analyzed for the presence of P-gp, GST-π and TOPO Ⅱ, by means of immunohistochemistry method and AI measured by TUNEL method. [Results] The result showed that the higher P-gp expression in noed tissue, the longer the survival. The expression of P-gp related to staging (P<0.05). The positive relationship existed between the strength of GST-π expression and the survial time. The expression of TOPO Ⅱ was associated with tissue differentiation(P<0.05). Apoptosis index related with clinical stage of gastric carcinoma. There was no relationship between MDR and apoptosis (P>0.05) [Conclusion] MDR expression and Al belong to normal cell’s events presented in both normal and tumor tissues. The different expression depends on their biologic behavior and the gene deformities. Single variable analysis is not enough to indicate prognosis
