记忆力缺损老年人向痴呆移行过程中的关联性负变化
Contingent negative variation in the aged with memory impairment and dementia tendency in the course of advance towards dementia
【摘要】 目的 探讨记忆力缺损老年人 (AMI)在向痴呆移行过程中关联性负变化 (CNV)的特征与趋势。 方法 将研究组和对照组行CNV和长谷川痴呆量表 (HDS)检查。 2~ 3年后对研究组中的 2 2例进行CNV和HDS复测。 结果 研究组命令信号 (S2 )开始至按键间的时间 (RT)和S2 后负变化 (PINV)的时程分别为 (2 84 3± 170 6 )ms和 (35 7 6± 2 0 4 7)ms,较对照组〔(149 8± 10 1 7)ms和(2 19 7± 135 5 )ms〕延长 (P <0 0 5或 0 0 1) ;期待波 (EW)波幅及其面积分别为 (15 3± 1 0 ) μV和 (74 6± 5 3 6 ) μV2 ,较对照组〔(2 1 1± 7 3) μV和 (112 0± 5 4 1) μV2 )〕缩小 (P <0 0 1) ;EW波形不规则、不稳定。 2~ 3年后复测 2 2例 ,HDS评分为 (19 6± 3 2 )分 ,较其第 1次〔(2 6 0± 3 8)分〕减低 (P <0 0 1)。CNV指标中RT为 (2 6 2 6± 71 6 )ms,较第 1次〔(2 10 1± 91 6 )ms〕延长 (P <0 0 5 ) ;PINV时程为 (175 3± 2 9 8)ms,较第 1次〔(32 1 5± 2 2 2 4)ms〕缩短 (P <0 0 1) ;EW的波幅和面积分别为 (14 3± 5 2 ) μV和(6 5 2± 40 0 ) μV2 ,PINV的波幅和面积分别为 (7 0± 4 0 ) μV和 (2 9 4± 46 3) μV2 ,均较第 1次缩小 (P<0 0 5或 0 0 1) ;EW波形不稳定与减低。 结论 CNV波幅
【Abstract】 Objective To investigate the feature and general trend of the contingent negative variation (CNV) in the aged with memory impairment (AMI) and dementia tendency in the course towards advanced dementia. Methods CNV in 69 AMI subjects (serving as the study group) and 22 healthy elderly subjects selected from well age matched (serving as the control group) were examined. Hasegawa’s dementia scale for the aged (HDS) in all AMI were also examined. 2 3 years later, 22 from the study group were examined again. The wave forms and relevant indexes of CNV were observed and analyzed. Results Reaction time(RT)〔(284.3±170.6) ms vs (149.8±101.7) ms, P <0.01〕and time of post imperative negative variation(PINV)〔(357.6±204.7) ms vs (219.7±135.5) ms, P <0.05〕was longer compared with those in the control group; the amplitude(B)〔(15.3±1.0) μV vs (21.1±7.3) μV〕and the area of the expectation wave (EW area) 〔(74.6±53.6) μV 2 vs (112.0±54.1) μV 2〕decreased. The scores of HDS in the latter were signifcantly decreased than that of the former(19.6±3.2 vs 26.0±3.8, P <0.01), the main indexes in the latter were significantly decreased compared to those in the former 〔PINV (175.3±29.8) ms vs (321.5±222.4) ms;B:(14.3±5.2) μV vs (19.4±8.0) μV;B’:(7.0±4.0) μV vs (13.6±7.8) μV; EW area: (65.2±40.0) μV 2 vs (107.8±75.5) μV 2; PINV area: (29.4±46.3)μV 2 vs (90.2±96.2) μV 2 〕. The EW in the latter was unstable and the amplitudes of their EW decreased too ( P <0.05 or 0.01). RT was prolonged and the whole wave of the CNV gradually decreased along the development of cognitive dysfunction. Conclusions A significant relationship was observed between the severity of cognitive dysfunction, and the reduction degree of the amplitude and the prolongation of RT on CNV. The CNV might be a valuable clue for the study of patho physiological function in the early stage of mental deterioration and useful for dynamically observing the course and degree of the cognitive dysfunction on AMI.
