近海石油平台工作人员海上与陆地24小时动态血压变化比较
Comparison of 24 hours ambulatory blood pressure variation in offshore oil workers during periods of working on platform and resting on land
【摘要】 目的 探讨近海石油生产作业体制及工作环境对工作人员血压的影响。方法 采用美国Welch Allyn Tycos无创性携带式动态血压监测仪 ,监测 30名健康海上平台工作人员在海上工作期间与陆地休息期间的中段某一天 2 4h动态血压。结果 将 2 4h动态血压分解为 2 4h平均血压、最大血压、最小血压、白天平均血压、夜间平均血压等分析指标。对比海上与陆地发现海上收缩压显著高于陆地收缩压 (最小 SBP除外 ) ,舒张压两者间差异不显著 ;海上血压负荷显著高于陆地 ;海上作业期间的心率与陆地休息时差异不显著。进一步以每小时为观察单位 ,发现无论是在海上还是在陆地 ,收缩压、舒张压及心率均呈日间上升、夜间下降趋势。 8∶ 0 0~ 12∶ 0 0时收缩压为海上高于陆地 ,但差异不显著 ,其余各小时均为海上显著高于陆地 ;舒张压除 2 3∶ 0 0时海上高于陆地外 ,其余各小时海上与陆地差异不显著。2 4h动态心率为 9∶ 0 0时陆地显著高于海上 ,18∶ 0 0、1∶ 0 0、2∶ 0 0、6∶ 0 0时海上显著高于陆地 ,其余差异不显著。结论 本研究结果提示近海石油生产的紧张工作环境可引起工作人员收缩压升高 ,但这种升高是暂时性的 ,一旦返回陆地放松后 ,即可恢复
【Abstract】 Objective To study the influence of working system and environment on the blood pressure of offshore oil production workersMethods 24 h ambulatory blood pressure of 30 healthy workers at a day in the middle of working on offshore platform and resting on land were monitored with an ambulatory blood pressure monitor made by Welch Allyn Tycos Company. 24 h ambulatory blood pressure was analyzed with different indexes including 24 h average blood pressure,maximum blood pressure,minimum blood pressure,and average day time and night time blood pressures.Results The systolic blood pressures during offshore period were significantly higher than those on land period(except for the minimum stystolic blood pressure),but no significant differences of diastolic blood pressures were found between the two periods.The blood pressure loads in the offshore period were significantly higher than those in the on land period,but the heart rates were of no significant differenceHourly analysis of the 24 h ambulatory blood pressures revealed a tendency of rising up of SBP,DBP and heart rate in the day time and falling down in the night time.The offshore SBPs were almost all significantly higher than those on land,except that,the differences were not significant during the 8∶00~12∶00 period.There were no significant differences of DBP between offshore and on land period,except that at 23∶00 offshore,it was significantly higher than that on land.The 24 h ambulatory heart rate at 9∶00 on land was significantly higher than that offshore;while at 18∶00, 1∶00, 2∶00 and 6∶00 offshore,they were significantly higher than those on land;as for the rest times,there were no significant differences between that offshore and on land.Conclusions The results indicated that the stress of working environment on offshore oil production platform might cause a temporal rising of systolic blood pressure during the working period,but it could fall down after going back and having rest on land.
