不同伤情下Ⅱd区屈肌腱修复方法的实验研究

【摘要】 目的比较Ⅱ d区内屈肌腱在单纯切割伤和严重挫伤的情况下仅修复深肌腱和深、浅肌腱同时修复的效果。方法将 24只来亨鸡随机分为 4组,每组 6只。以鸡双足中趾Ⅱ d区为手术区,每组均切除腱鞘。 A组,单纯切割伤,深、浅肌腱同时修复; B组,单纯切割伤,仅修复深肌腱; C组,严重挫伤,深、浅肌腱同时修复; D组,严重挫伤,仅修复深肌腱。肌腱修复后 6周末处死动物,对鸡趾进行生物力学评定 (滑动距离和屈曲功 )和形态学评定 (粘连的大体程度和表面积 )。结果Ⅱ d区屈肌腱损伤后,单纯切割伤深、浅肌腱同时修复与仅修复深肌腱效果相当,在肌腱的滑动距离、屈曲功及粘连的大体程度、表面积上差异无显著性意义 (P >0.05);严重挫伤时,仅修复深肌腱的效果优于深、浅肌腱同时修复,粘连的表面积小于深、浅肌腱同时修复组,差异有显著性意义 (P< 0.05)。结论Ⅱ d区内屈肌腱单纯切割伤时可行深、浅肌腱同时修复;而在严重挫伤时,应该做深肌腱修复,浅肌腱切除。

【Abstract】 Objective To evaluate the results of repair of both tendons or profundus tendon alone in clean cut injuries as well as the severe traumatic injuries of the flexor tendons in zoneⅡ d. Methods Twenty four leghorn chickens were divided into 4 groups at random. Tendon sheath was resected after flexor tendons repaired in zoneⅡ d of long toes. In Group A, clean cut tendon injuries were produced and flexor digitorum superficialis(FDS) and flexor digitorum profundus(FDP) tendons were repaired. In Group B, clean cut tendon injuries were produced and only FDP tendon was repaired. In Group C, severe tendon injuries were produced and both FDS and FDP tendons were repaired. In Group D, severe tendon injuries were produced and only FDP tendon was repaired. Biomechanical studies of gliding excursion and flexion function were carried out at 6 weeks after tendon repair. The extent of adhesion was examined. Results There were no significant differences in tendon excursion and flexion function of the toes between groups A and B. Excursions of FDP tendons in group D were statistically larger than those in group C(P< 0.05). Flexion function and extent of adhesion in group D were significantly less than those in group C(P< 0.05). Conclusion This study demonstrated that both FDS and FDP tendons could be repaired in clean cut injuries and only FDP tendon should be repaired in severe traumatic injuries in zoneⅡ d.
