股骨质量对人工股骨头置换术后股骨应力改变的生物力学影响
Biomechanical study of the effect of femoral quality on femoral stress change after femoral head prosthesis replacement
【摘要】 目的 探讨股骨质量对人工股骨头置换术后股骨应力改变的影响。方法 对 10具尸体 20侧股骨标本分别行非骨水泥有肩托( non-cemented collared, NCC)、非骨水泥无肩托( non-cemented non-collared, NCNC)、骨水泥有肩托( cemented collared, CC)、骨水泥无肩托( cemented non-collared, CNC)假体置换。在生物力学试验机上行轴向 1 000 N载荷下股骨应变测定。结果 随着骨质量的减少,股骨应变有增加的趋势 (P0.05)。在同样的骨质量条件下,股骨大转子区域的股骨应变表现为: NCNC >NCC >CNC >CC假体,股骨矩区域股骨应变表现为: NCC >NCNC >CC >CNC假体,杆端区域股骨应变表现为: CC=CNC >NCC >NCNC假体。在不同骨质量情况下具有相同的规律( P >0.05)。在杆端区域,骨水泥假体的应变稍大于非骨水泥假体,而在其他区域,骨水泥假体的应变普遍较小。正常骨质组与骨质疏松组的应力遮挡和应力集中比较,差异有显著性意义( P
【Abstract】 Objective To study the influence of femoral biological quality on femoral stress change after femoral head prosthesis replacement. Methods Cemented collared(CC), cemented non-collared(CNC), non-cemented collared(NCC), non-cemented non-collared(NCNC) femoral head prosthesis replacement were performed in ten cadaver femur specimens; and femoral strains were determined with 1 000 N axial load. Results Femoral strains increased as bone mineral density decreased(PNCC >CNC >CC, point‘ B’ of calcar as: NCC >NCNC >CC >CNC, point‘ C’ of stem tip zone as: CC=CNC >NCC >NCNC. Different bone quality groups had identical tendency( P >0.05) . Strains of cemented groups were usually less than that of non-cemented ones for all zones except the stem tip zone.
