【摘要】 目的　评价超分割放射治疗食管癌的长期疗效及放射反应和并发症。方法　 92例食管癌随机分为 2个组 :常规分割放射治疗组 46例 ,1次 /d ,2 .0 0Gy/次 ,5次 /周 ,总剂量 70 .0Gy ,35次 ,47～ 5 6d ;超分割放射治疗组 46例 ,2次 /d ,1.15Gy/次 ,间隔 4～ 6h ,5d/周 ,总剂量 80 .5Gy ,70次 ,49～ 5 3d。全部病例均采用60 Co照射。结果　 2个组急性和晚期放射反应差异无显著意义 (P >0 .0 5 )。 5 ,10年局控率超分割组优于常规组 (P <0 .0 5 ) ,分别为 5 1.7%,41.8%及 2 8.4%,2 0 .3%。3,5 ,7,10年生存率超分割组与常规组分别为 2 8.3%,2 3.9%,2 1.7%,17.3%及 2 1.7%,17.4%,13.0 %,10 .9%,2个组差异无显著意义 (P >0 .0 5 )。结论　食管癌超分割放射治疗的局控率优于常规放射治疗 ;长期生存率之差异无显著意义 ;患者能很好耐受超分割放射治疗 ,长期生存者无晚期并发症 ,生存质量良好。更多还原

【Abstract】 Objective To evaluate the effect of hyperfractionation radiation for esophageal carcinoma. Methods Ninety two patients with esophageal carcinoma were randomized into two groups. The conventional fractionation (CF)group received routine radiotherapy 2.00?Gy per day, 5 times a week to a total dose of 70.0?Gy in 47～56 days. The hyperfractionation radiation(HF) group received the radiotherapy 1.15 ?Gy twice a day with an interval of 4 to 6 hours between fractions to a total dose of 80.5?Gy in 49～53 days. Both groups were treated by 60 Co γ rays.Results There were no significant difference in acute and late responses between two groups (P>0.05).The 5 ,10 year local tumor control rates of HF group were significantly improved when compared to CF group, being 51.7%,41.8% vs 28.4%,20.3%,respectively (P<0.05). The 3 ,5 ,7 ,10 year survival rates were 28.3%,23.9%,21.7%,17.3% in HF group and 21.7%,17.4%,13.0%,10.9% in CF group (P>0.05).Conclusions Local control rate of hyperfractionation radiotherapy of esophageal carcinoma is better than conventional radiotherapy,however there is no significant difference in survival rate between two groups. It is well tolerated, wihtout late complications in long term survivors.更多还原
