高能震波治疗骨不连接的实验研究
Nonunion treated with high-energy shock waves in rabbits
【摘要】 目的 观察高能震波 (highenergyshockwaves,HESW)对骨愈合的影响 ,探讨促进骨折愈合的新方法。 方法 手术造成 5 0只家兔双侧胫骨中段 6mm骨缺损后用外固定器固定。 12周后选择经X线片检查证实的 6 8侧胫骨骨不连接 ,随机分成两组进行研究。治疗组 34侧 ,对其胫骨骨折部施以HESW治疗 (14.5kV ,2 0 0 0次 ) ;对照组 34侧 ,不加任何治疗。术后 2 ,6 ,12周进行摄片、组织学及透射电镜检查。 结果 对照侧均为骨不连接 ,治疗组除 5只仍为萎缩性骨不连接外 ,均获得骨愈合 (P <0 .0 1)。 结论 HESW能够刺激骨痂形成 ,促进骨折愈合 ,可望成为一种可靠的、非手术治疗骨不连接和骨折延迟愈合的新方法。
【Abstract】 Objective To explore the effect of high-energy shock waves (HESW) on fracture healing in order to seek for a new treatment for nonunion. Methods Segmental defects of 6 mm were created in bilateral tibial diaphyses of 50 rabbits and then distracted by external fixation. At 12 weeks after operation, 68 tibias were confirmed as nonunion on the basis of radiographic findings. The 68 tibias were randomly divided into two groups, 34 tibias in the experimental group treated with shock waves of 14.5 kV for 2 000 times, and the others served as the control group, with no treatment. X-ray films and histological sections were made to evaluate the effect of fracture healing after 2, 6 and 12 weeks, respectively. Results Bone union was radiographically observed on 17 out of 22 tibias in the experimental group at 12 weeks after shock wave treatment. The entire control group had radiographically persistent nonunion at the termination of the study. Statistical analysis with χ 2 test ( P <0.01) demonstrated the significance in outcome between the two groups. Conclusions It suggests that HESW can stimulate callus formation and accelerate fracture healing. Therefore, it may become a new reliable and nonsurgical treatment for delayed and nonunion fractures.
