远立体视测定对间歇性外斜视手术时机选择的临床观察

【摘要】 目的 探讨间歇性外斜视最佳手术时机 ,以求达到矫正斜视并获得完善的双眼单视功能目的。方法 采用颜氏远用和近用随机点立体图 ,对 69例间歇性外斜视进行手术前后立体视锐度测定 ;用三棱镜法测量远近斜视角 ;用同视机法测定双眼单视功能。结果  69例间歇性外斜视中术前 4例通过远立体视 ,4 7例通过近立体视 ,二者有高度显著性差异 (P <0 0 0 1 ) ;术后 37例通过远立体视 ,与术前比较有高度显著性差异 (P <0 0 0 1 ) ,而近立体视手术前后无明显差异 ;病程愈长 ,术后获得立体视者愈少 ,病程短则相反。结论 通过测定远近立体视 ,合理掌握间歇性外斜视手术时机 ,对其预后估计及疗效评价有重要意义。

【Abstract】 To study the optimal time for operation of intermittent exotropia, so that we can orthopia strabismus and attain normal binocular function. Method Using the Yan′s far and near random dot stereogram to examine 69 intermittent exotropia patient′s stereoacuity before and after the operation. Using the prism to examine the far and near of strabismus. Using the synoptophore to examine the binocular function. Result Among the 69 intermittent exotropia patients, only four had passed the far stereopsis before the operation and 47 had passed the near stereopsis. This difference between the two groups was statistically significant (P<0 001). After the operation, 37 patients had passed the far stereopsis and it was statistically significant different (P<0 001) compared with that of pre operation. There was no statisticaly significant difference between the pre oeration group and the post operation group concerning the near stereopsis. Patient with long course disease was more difficult to attain the stereopsis than that with shorter disease duration. Conclusion Examining the far and near stereoacuity is of benefit to determine the optimal time of the intermittent exotropia operation, and is valuable for evaluating the therapeutic effect.
