高血压病各期脑血液动力学TCD监测

【摘要】 目的 :探讨高血压病各期脑血液动力学变化与临床关系。方法 :利用 3D- TCD技术检测颈内动脉 (ICA) ;大脑中动脉(MCA) ;大脑前动脉 (ACA) ;大脑后动脉 (PCA) ;基底动脉(BA) ;双侧椎动脉 (VA)的血流速度、脉动指数 (PI值 )、谱态与高血压病各期的相关性。结果 :8种异常共 2 31次发生在 2 1 3例高血压 期病人 ,1 0种异常共 46 0次发生在 2 2 0例 期病人 ,和 9种共 1 2 3次异常发生在 2 8例 期病人。同时有某些特殊的异常只发生在 期病人中。 CT断层显示 期高血压患者有脑梗塞 2例 , 期有 6例 , 期有 1 5例 ,说明脑血管的血流动力学的变化和脑血管意外呈相关性。结论 :高血压病的脑动脉的血液动力学异常与临床分期有明显相关性。 3D- TCD对脑血管受损程度及定位上有意义 ,且其易行及无创性

【Abstract】 Aim:To investigate the relationship between brain hemodynamics and clinical syndrome in hypertension patients Methods:Brain blood vessel comprising of internal corotid artery,middle cerebral arteries,anterior and posterior cerebral arteries, cerebral basical artery,and two side vetebral arteries were examined by 3D TCD in 461 cases of hypertension. Blood velocity,pulse index,and spectrum image in blood vessels were recorded and compared with 120 cases normal healthy. Results:In hypertension patients,8~10 different types of abnormality was shown in TCD. The occurrence rate of abnormalities was 231/213 in patients of phaseⅠhypertension,460/220 in patients of phase Ⅱ,and 123/28 in patients of phase Ⅲ,although some specific abnormalities was found in Ⅲ phase hypertension uniquely. CT image of cerebral infartion was found of 2 cases in phase Ⅰ,6 cases in phaseⅡ, and 15 cases in phase Ⅲ hypertension. The brain damages seems to be correlated with cerebral vessels hemokinetic abnormalities. Conclusion:3D TCD may determine the hemodynamic changes of cerebral vessels non invasively in hypertension patients.
