多普勒超声心肌作功指数在评价急性心肌梗死患者左室功能中的意义

【摘要】 目的 :探讨利用多普勒超声心肌作功指数评价左室功能的价值。方法 :急性心肌梗死患者 72例 ,其中 Killip分级 - 级 5 4例 , - 级 18例 ,正常对照组 44例。测定左室射血分数 ( EF)、每搏量 ( SV)、心脏指数 ( CI)、射血前期 /射血时间 ( PEP/ ET)、等容舒张期 ( IRT)、等容收缩期 /射血时间 ( ICT/ ET)及心肌作功指数( MPI)。 MPI=( ICT+IRT) / ET,由二尖瓣关闭至开放时间减去 ET所得值再除以 ET而得。结果 :MPI对照组为0 .49± 0 .0 8,Killip分级 - 级组为 0 .6 8± 0 .14, - 级组为 1.0 0± 0 .19,各组间均具有显著性差异 ( P<0 .0 1)。 MPI与心率无关。结论 :MPI是综合评价左室收缩和舒张功能的简便而准确可信的多普勒超声新指标

【Abstract】 Objective:To explore the value of Doppler index of myocardial performance(MPI) in patients with acute myocardial infarction(AMI) in the determination of left ventricular function Methods:The left ventricular function indexes(EF,SV,CI,PET/ET,IRT,ICT and ICT/ET)of 44 normals,54 patients with AMI in killip class I or II and 18 patients with AMI in Killip class Ⅲ or Ⅳ were measured using conventional echo Doppler methods Myocardial performance index〔MPI=(ICT+IRT)/ET〕was obtained Results: The index was easily measured The mean value of the index was significantly different between normal, Killip class Ⅰ or Ⅱ and Killip class Ⅲ or Ⅳsubjects (0 49±0 08,0 68±0 14 and 1 00±0 19, respectively, P <0 01 for all comparisons) Within functional groups,the value of the index did not appear to be related to heart rate Conclusions:(ICT+IRT)/ET is a conceptually new, simple and reproducible Doppler index of combined systolic and diastolic myocardial performance in patients with primary myocardial systolic dysfunction
