经颅多普勒超声监测脑受压减压后脑血流改变的实验研究
Changes of Cerebral Blood Flow in Response to Brain Swelling after Decompression
【摘要】 目的 :探讨单纯脑受压减压后脑肿大与相应脑血流参数改变的关系。方法 :硬膜外水囊加压法复制出兔急性颅内高压模型 ,同时监测兔颅内压及基底动脉血流 ,Vd≈ 0时停止加压并维持颅内压于此水平。A、B、C三组动物维持时间分别是 30、6 0、12 0分钟 ,然后减压并观察颅内压及脑血流至减压后 2 4小时 ,处死动物取脑送病理检查 ,SSPS统计软件包处理数据。结果 :A、B组动物减压后 4小时内脑血流恢复正常 ,B组动物 2 0 %发生脑水肿 ;C组减压4小时 Vd、RI均较减压 30分钟相应值有显著改变 (P<0 .0 1) ,10 0 %发生脑水肿。结论 :单纯脑受压减压后 4小时脑血流不恢复则发生脑水肿可能性极大 ,宜采取积极措施降低颅内压
【Abstract】 Objective:To investigate the relation between brain edema and swelling after decompression of simple brain compression and simultaneous changes of the cerebral blood flow.Methods:A steady acute step increasing intracranial pressure model was created by fluid positive pressure irrigation into a epidural tiny bag beneath the skull. Rabbits were randomly divided into 3 groups. Intracranial pressure(ICP) was maintained steady for 30, 60, 120 minutes respectively in group A, B, C while the Vd≈0. Then the water in the bag was drained out. Meanwhile the changes of the ICP and cerebral blood flow (CBF) of basilar arery (BA) were investigated from the beginning of the experiment to the end of 24 hours after decompression. Then the animal was killed and it’s brain was taken for pathological examination. Data were analyzed with SSPS. Results:CBF reached normal in group A and B within 4 hours after decompression. In group C, ICP increased after decompression, 4 hours after decompression, the values of Vd and resistance index (RI) changed signficantly compared with the values that were gained 30 minutes after decompression respectively (P=0 0062,0 0018). All of the animals developed a brain edema or/and swelling.Conclusion:It indicated that in simple drain comrpession, if Vd (RI) did not reached normal 4 hours after decompression and even decreased (increased) gradually, brain edema or/and swelling would be always unaviodable.
