肺静脉及二尖瓣血流估测肺小动脉楔嵌压与心导管检测对比研究
A Study of Pulmonary Wedge Pressure Estimation by Pulmonary and Mitral Blood Flows Compared with by Cardial Catheterization
【摘要】 目的 :探讨多普勒超声检测肺静脉及二尖瓣血流参数估测 PWP与心导管测得 PWP相关性及实用价值。方法 :采用 Pozooli等研究 ,利用线性回归分析 ,获得肺静脉及二尖瓣血流速变等参数与 PWP的相关关系 ,导出三个估测 PWP的最相关方程 3公式计算 PWP,对 36例慢性心衰心脏病患者同步行心导管检测 PWP作直线相关分析。结果 :多普勒超声心动图与右心导管实测 PWP作直线相关分析 ,求得两者呈非常显著相关 ,其相关系数 r=0 .86 (p<0 .0 0 1)。结论 :多普勒超声检测参数及多元回归分析 ,能可靠地估测其 PWP值。这种简便、易行、无创方法应用于心力衰竭患者的追踪及监测有临床实用价值。
【Abstract】 Objective:To investigate the correlations and practical values in determination of PWP by Doppler measurements of pulmonary venous flow and mitral flow and by cardiac catheterzation Methods:By using method of Pozooli and linear regression analysis,we obtain the parameters of pulmonany venous flow velocity and mitral flow velocity and the correlations with PWP,The deduced three formulas showed the strongest correlations in determinations of PWP. Simultaneous Doppler measurements and right heart catheterizations were performed in 36 patients with chronic heart failure,then calculated PWP and analysed linear correlativity Results:analysed linear correlativity of the actual PWP gotten by Doppler measurements and right heart catheterizations, the hightest correlation was obtained (r=0 86,P<0 001) Conclusion:According to multilinear rcgression analysis,PWP can be reliably estimated by parameters of Doppler measurements This noninvasive method which can be easy performed is simple and convenient,especially for the monitor and follow up of the patients with heart failure
