检测细胞角蛋白19片段和腺苷脱氨酶对良、恶性胸水鉴别诊断的意义

【摘要】 目的:联合检测血清和胸水中细胞角蛋白19 片段(CYFRA21- 1)和腺苷脱氨酶(ADA)活性,旨在探讨其对良、恶性胸水的鉴别诊断意义。方法:用免疫放射法(IRMA) 检测血清及胸水CYFRA21-1 并用Giusti 改良法检测ADA活性。结果:非小细胞肺癌(NSCLC) 组胸水CYFRA21- 1 较结核组及小细胞肺癌(SCLC) 组明显增高( P< 0.01);NSCLC组胸水CYFRA21- 1 较血清CYFRA21- 1 亦明显增高( P< 0.01);结核组ADA胸水/血清比值(ADAp/s) 较肺癌组显著增高( P<0 .01)。联合检测胸水CYFRA21 -1 及ADAp/s 对肺癌诊断的敏感性为100% ,特异性为83% ,准确率为91 % 。结论:联合检测胸水和血清CYFRA21 -1 及ADA活性对良、恶性胸水鉴别诊断有一定价值。

【Abstract】 Objective:To measure CYFRA 21-1 levels and adenosine deaminase activity in serum and pleural fluid.To aim at investigating the roles in differentiation diagnosis of benign and malignant pleural fluid.Method:To measure CYFRA 21-1 levels in serum and pleural fluid by IRMA,and to measure adenosine deaminase activity in serum and pleural fluid by the modified Ginsti assays.Result:CYFRA 21-1 levels of non small lung cancer (NSCLC) group were higher than those of tuberculosis group and small cell lung cancer (SCLC) group in pleural fluid (P<0.01);CYFRA 21-1 level of pleural fluid was higher than that of serum in NSCLC group;The ration of pleural fluid to serum for ADA(ADAp/s) in tuberculosis group were higher than that in lung cancer group (P<0.01).If CYFRA 21-1 levels and ADAp/s in pleural fluid were measured simultaneously for differentiation diagnosis of lung cancer,the sensitivity rates were 100%,the speciticity rates were 83%,the accuracy rates were 91%.Conclusion:It had potential value to measure CYFRA 21-1 levels and adenosine deaminase activity simultaneously for differentiation diagnosis of benign and malignant pleural fluid.
