三维CT重建对跟骨骨折治疗的临床意义
Value of Three-Dimensional Computed Tomographic Reconstruction in Treatment of Calcaneal Fracture
【摘要】 目的 :评价跟骨CT二维图像的三维重建对跟骨骨折治疗的临床意义。材料和方法 :取10例患者的12只跟骨CT二维图像 ,包括10例跟骨骨折 ,2例正常跟骨。通过图像处理技术 ,对跟骨进行独立三维图像重建。CT扫描设备采用Elscient2400E。结果 :可以直接显示单独的跟骨立体图像 ,骨折块相对位移的情况 ,跟骨体变低、变宽的程度 ,尤为重要的是可以直接显示跟距关节面被破坏的程度 ;还可以在屏幕上移动骨块对骨折进行模拟复位 ,设计内固定的置放位置、攻入方向 ,从而实现模拟复位、内固定手术治疗 ,帮助达到解剖复位。结论 :三维CT对跟骨骨折的诊断以及治疗方案的选择有一定的参考价值。
【Abstract】 Purpose: To evaluate the clinical significance of three-dimensional(3-D)computed tomographic reconstruction in the treatment of calcaneal fractrue. Materials and Methods: CT examinations of 12 calcanea from 10 patients were performed on a Elscient 2 400E CT Scanner including 10 calcaneal fractures and 2 normal calcaneal. The 3-D reconstruction was obstained from the raw data of 2-D images with a CT data processing system. Results: Joint alignment, Displacement of fragments, and the integrity of the medial wall are key factors in deciding whether surgical reconstruction is indicated and what surgical approach to use. Examples of such information obtained from patients with calcaneal fractures are reported comparing 3-D CT reconstruction to CT scan results and conventional roentgenograms. We can move the fragments, set the position and the direction of the K-wire on the screen, help the doctor to get an anatomic reduction. Conclusion: 3-D CT is a useful technique for the diagnosis and surgical planning of calcaneal fractures.
