间歇性外斜视术后双眼视功能重建的临床研究
A clinical study of binocular vision rebuilding after surgery in intermittent exotropia
【摘要】 目的 :了解间歇性外斜视术后双眼视功能重建的临床规律。方法 :对 1 0 0例术后建立双眼视觉的患者分成 4组 ,对术前术后双眼视觉进行分析比较。结果 :( 1 ) 1 0 0例中术前有不同程度的近立体视觉者 67例 ,术后增加至 93例 ,术后近立体视觉重建有明显改善 (P <0 0 5)。 ( 2 )术前有同视机Ⅰ级功能者 51例 ,术后增加至 82例 ,术后同视机Ⅰ级功能重建有显著改善 (P <0 0 5)。 ( 3)术前有同视机Ⅱ级功能者 4 0例 ,术后增加至 91例 ,术后同视机Ⅱ级功能重建有显著改善 (P <0 0 5)。 ( 4)组 1组 2术前术后立体视敏度程度分布无显著性差异 (P >0 0 5)。 ( 5)组 1组 3术前术后融合范围大小的比较无显著性差异 (P >0 0 5)。结论 :( 1 )手术对间歇性外斜视双眼视觉功能的重建有显著疗效。 ( 2 )术前同视机融合功能损失最早 ,间歇性外斜视手术时机以同视机融合功能损失为指征。 ( 3)融合功能发育不全是间歇性外斜视发病机制的主要因素。
【Abstract】 Objective:To understand the rule of the rebuilding binocular vision in intermittent exotropic patients after strabismus surgery.Methods:It was divided into four groups for 100 cases who have builded binocular vision after strabismus surgery and it was analysed before and after strabismus surgery.Results:1 It had different degree near steroacuity in 67 cases before strabismus surgery and it was increased to 93 cases after strabismus surgery.Rebuilded near steroacuity was prominent after strabismus surgery(P<0 05).2 It had synoptophore Ⅰ degree in 51 cases before strabismus surgery and it was increased to 82 cases after strabismus surgery.Synoptophore Ⅰ degree was rebuilded prominent after strabismus surgery (P<0 05).3 It had synoptophore Ⅱ degree in 40 cases before strabismus surgery and it was increase to 91 cases after strabismus surgery.Rebulided synoptophore Ⅱ degree was prominent after strabismus surgery (P<0 05).4 Distribution of near steroacuity degree was not prominent before and after strabismus surgery in group 1 and group 2 (P>0 05).5 Comparison of fusion area was not prominent before and after strabismus surgery in group 1 and group 3 (P>0 05).Conclusions:1 The surgery has prominent effection for binocular vision rebuilding in intermittent exotropia.2 Synoptophore fusion lost early before strabismus surgery and the damage of synoptophore fusion was the indication of intermittent exotropia surgery.3 The unsoundness growth of fusion function was the major pathogenetic factor in intermittent exotropia.
