喉返神经-膈神经吻合术后声带外展肌的神经肌电图表现

【摘要】 目的 研究声带外展肌 -环杓后肌在喉返神经 -膈神经吻合术前、后的神经肌电图表现。方法 造成犬单侧喉返神经完全性损伤 ,即行单侧犬喉返神经 -膈神经吻合术 ,对吻合术前、后同侧环杓后肌神经电图各项参数进行对比分析。结果 吻合术前环杓后肌的神经电图表现为 :潜伏期 1 2 6± 0 86ms,神经传导速度 15 0 8±0 47ms s ,幅值 5 0 49± 4.75mV。神经吻合术后 4个月 ,环杓后肌神经电图出现清晰动作电波形 ,表现为潜伏期延长 ,传导速度减慢 ,动作电位峰 -峰值明显减小。结论 神经吻合术后 4个月 ,出现环杓后肌肌纤维不同程度的神经再支配 ,但新生神经末稍终板数量及神经鞘膜之再生还未完全达到正常水平

【Abstract】 Objective To study the changes of electro-neurogram of the posterior cricoarytenoid muscle in the reinnervation between recurrent laryngeal nerve and phrenic nerve.Methods The reinnervation between the recurrent laryngeal nerve and the phrenic nerve was established immediately after the section of the recurrent laryngeal nerve.Eletro-neurogram was used before and after the nervous reinnervation for analysing the data.Results The normal electro-neurogram of the posterior cricoarytenoid muscle before reinnervation are:nervous latency:(1.26±0.86)ms,amplitude:(50.49±4.75)mV,conduction speed:(15.08±0.47)m/s.Four months later,the nervous latency was prolonged,the conduction speed was slower and the ampiltude was remarkably reduced.Conclusion Four months after the reinnervation,the action potential wave of the posterior cricoarytenoid muscle appeared clearly but the amplitude was reduced ,and the latency was prolonged.It meant that the muscle fibers were partially reinnervated but the amount of new nerve endings and the recovery of sheath of the nervous fibers did not reach the normal level.
