外周血与卵泡液中血管紧张素II和肾素活性在促超排卵周期中的变化

【摘要】 本研究通过对促超排卵患者血及卵泡液中肾素活性和血管紧张素 II浓度变化 ,探讨卵巢源肾素 -血管紧张素系统对卵泡发育的影响。研究对象为接受体外受精 -胚胎移植患者 ,共 2 0个治疗周期。分别于人绒毛膜促性腺激素 (h CG)注射日、取卵日、移植日采血 ,取卵日收集卵泡液 ,测定雌二醇、孕酮、睾酮、血管紧张素 II(ANG II)浓度及肾素活性 (RA)。结果 :取卵日和移植日血 RA、ANG II明显高于 h CG注射日 ;卵泡液各激素水平明显高于外周血浓度 ;卵泡液、外周血 RA与 ANG II均呈直线正相关 ,卵泡液 ANG II与孕酮 (P)呈显著负相关 ,与所取出的 III级以上卵母细胞数目呈显著正相关。结果表明 ,外源应用 h CG可以激活卵巢肾素 -血管紧张素系统 ,促超排卵周期卵泡液中高水平的RA、ANG II可能与卵泡发育、卵母细胞成熟有关。

【Abstract】 Objective: To test the regulation of ovarian renin angiontensin system (RAS) to follicular development. Methods: We studied 20 controlled ovarian hyperstimulation cycles (COH) of in vitro fertilizatioin and embryo transfer (IVF ET) patients who had regular menstrual cycles and had fallopian tube disease. Blood samples were collected on the day of injecting hCG, retrieving ova and embryo transfer. Follicular fluid were obtained on the day of retrieving ova. All specimens of blood and FF were stored for measure estradiol(E 2), progesterone (P), testosterone (T), renin activity (RA), angiotensin II (ANG II). Results: The levels of plasma RA and plasma ANG II in peripheral blood were higher on the day of retrieving ova and embryo transfer than on the day of injecting hCG; the levels of various hormones in follicular fluid were higher than those in blood. Both follicular fluid RA and plasma RA had postitive correlation with follicular fluid ANG II and plasma ANG II, but follicular fluid ANG II had negative correlation with follicular fluid P and positive correlation with grade III oocyte. Conclusion: We suggest that extrogenous hCG can stimulate ovarian RAS, and ovarian RAS might play an important role in follicular development and oocyte maturation.
