全天血糖谱的临床应用
Clinical application of blood glucose profile
【摘要】 目的 设计检测全天血糖谱以客观反映病人血糖的动态改变。方法 1型、2型及初发病人共386例。新入院病人当天测全天定时血糖谱 ,每隔 4天测 1次 ,据此调整药物。根据血糖谱归类 ,分析。结果 根据全天血糖谱 ,糖尿病病人血糖大致分成“完全未控制型”占 33.6 % ,“餐后高血糖型”占 31.4% ,“黎明高血糖型”占 12 .9% ,“日间高血糖型”占 12 .7% ,“反应性高血糖型”即“Somogyi效应型”占 9.4%。各型都可能与其不同的治疗背景相关。同时 ,全天血糖谱使低血糖发现率增加 ,发现低血糖占 2 0 .7% ,尤以无症状居多 ,占低血糖反应中的 87%。结论 应用全天血糖谱可较准确地估计病人的血糖控制情况 ,也能及早发现无症状性低血糖。
【Abstract】 Objective For evaluating the accuracy of therapy, we designed a 24 hour blood glucose profile as a guide of medication for diabetics. Methods Both type 1 and type 2 inpatients were recruited in our department during April 1996 and March 1998. The 24 hour blood glucose profile included that determined before and 2 hours after the three meals and before sleep which was done after admitted to the hospital and then every 4 days for modification of drug dosages. Results Basing on this, the blood sugar pattern of the diabetic patients could be divided into five types, i.e. totally uncontrolled (33.6%), postprandial hyperglycemia (31.4%), hyperglycemia at dawn (12.9%), daytime hyperglycemia (12.7%), and Somogyi effect (9.4%). Each type could be related to the treatment background. Concomitently, the rate of discovery of hypoglycemia was found to be 20.7%, especially the symptomatic ones which accounted for 87% of the total hypoglycemic cases. Conclusion This profile is good for assessment of blood sugar control and early discovery of asymptomatic hypoglycemia. (Shanghai Med J, 2000,23∶672 674)
