尼尔雌醇和利维爱对绝经后妇女骨密度的影响

【摘要】 目的 对 10 4例绝经后妇女应用尼尔雌醇与利维爱后骨密度的变化进行比较性的分析。方法 将年龄 47~ 5 8岁已绝经 2~ 10年妇女随机分为两组 :A组 6 7例口服尼尔雌醇 2 .5mg ,每 2周 1次 ,用 3个月后在最后 1个月加用安宫黄体酮 10mg ,每天 1次 ,共 10~ 14天。B组口服利维爱 1.2 5mg ,每天 1次。两组均于用药前 ,服药后 3个月、6个月、12个月用美国LUNAR公司生产的Achilles +跟骨定量超声分析仪测定跟骨硬度指数 (SI)。结果 通过两组比较发现A组服药后 3个月 ,SI上升缓慢 ,服药后 6个月骨量增加但低于B组。B组在服药后SI迅速上升 ,两组比较P值 <0 .0 1。结论 尼尔雌醇虽然可以改善更年期症状 ,但不能迅速提高骨密度。利维爱不仅可以明显抑制骨质丢失 ,并可以增加骨密度的含量 ,对低骨量有明显效果

【Abstract】 Objective To observe the influence of nylesteriol and livial on bone mineral densiry in 104 postmenopausal women through comparative analyses. Methods The postmonopause elderly women; Age Range from 47~58 years, 2 to 10 years after monopause, were randomly divided into two groups. Group A, 67 patients, received nylesterio; 2.5 mg per two weeks for three successive months, and medroxyprogesterone 10 mg/day was added for 10~14 days afterwards. Group B 37 patients livial 1.25 mg/day. The BMD of the heel bone were measured before and after medication at 3、6、12 month using the Achilles heel bone quantitative ultrasound USA. Results Through comparison between the two group, we found that the stiffness index in group A increased slowly at 3 rd month. Though the SI went up at 6.12 th month, it was lower than that of group B. While in Goup B, The SI increase rapidly. There is signicicant difference between the two groups (<0.01). Conclusion Although nylesteriol can improve the climacteric symptoms rapidly, the speed of its increase in BMD is slow whereas livial can increase the BMD quickly and inhibit bone loss markedly. It has prompt and apparent effect on low bone mass. (Shanghai Med J, 2000,23∶547 549)
