18氟-脱氧葡萄糖PET全身显像在探测肿瘤原发灶中的应用

【摘要】 目的 对 17例病理组织学证实的转移性肿瘤患者进行18氟 脱氧葡萄糖 (18F DG)PET全身显像 ,以评价其在寻找原发灶中的价值。方法 所有患者均进行18F DGPET全身显像 ,结果判断应用目测法和半定量分析方法 (标准摄取值 ,SUV)。结果  17例患者中 ,12例PET可检出原发灶 (70 .6 % ) ,其中肺癌 9例 ,鼻咽癌 2例 ,乳腺癌 1例。常规显像检查只有 6例发现原发灶 (35 .3 % )。 5例原发灶不明者 ,临床随访 2~ 10个月 ,1例证实为前列腺癌 ,余 4例临床随访未见原发灶。 17例患者中 8例PET检出了常规显像检查未能发现的其他转移灶。结论 对转移瘤患者进行全身PET检查有助于原发灶的寻找 ,尤其是临床上原发灶不明者 ,PET一次检查可了解肿瘤原发灶及全身转移情况 ,包括骨骼及软组织的转移 ,为临床制订治疗方案提供可靠依据

【Abstract】 Objective It is very important to localize the primary focus of the tumor for guiding biopsies and selecting the appropriate treatment protocols. The aim of this study was to evaluate the FDG PET in detecting the primary focus of cancer. Methods 17 patients with various metastatic cancer were included, They were as follows: cerebral metastases in 5, skeletal metastases in 4, cervical lymph nodes in 4, pleural effusion in 2, axillary lymph node metastasis and sulcntaneous metastatic nodule at the back in one each. All patients underwent whole body 18 F DG PET imaging. The images were interpreted by visual inspection and semi quantitative analysis (standardized uptake value, SUV). Results FDG PET was able to identify the primary focus of the tumor in 12/17 patients (70.6%). Among them, 9 were in the lung, 2 in nasopharynx, 1 in breast, whereas the conventional imaging method only detected primary focus in 6 patients. During the clinical follow up of 2 to 10 months, the primary tumor was found to be prostatic cancer in only 1 patients. Conclusion Whole body 18 F FDG PET is valuable in detecting the primary focus of the tumor, especially in patients with carcinoma of unknown primary. PET imaging allows identification of the primary site and metastatic lesions, including bony and soft tissue metastases in a single examination. FDG PET is helpful in selecting appropriate management of these patients. (Shanghai Med J, 2000,23∶518 521)
